Objective:The potentially inappropriate medication(PIM)in the elderly is a kind of drug that should be avoided in the process of using drug adverse reactions.The Beers standard is used to assess the potential for inappropriate use of drugs in older patients commonly.At present,the Beers standard is widely used for the evaluation of potential inappropriate medication in elderly patients.However,there are few evaluations based on the latest edition of Beers standard(2015)at home and abroad.The potential inappropriate use of drugs in the elderly patients in the traditional Chinese medicine hospital affiliated to college of Xinjiang medical university is not clear.Therefore,according to the Beers standard(2015),the drug use of the elderly inpatients in this hospital is evaluated retrospectively,in order to know the incidence of potential inappropriate drug use,drug distribution and related factors in elderly patients,and to provide reference for clinical safety of drug use in elderly patients.Methods: A total of 497 medical records of elderly patients aged ? 65 years old from affiliated TCM Hospital of Xinjiang Medical University were randomly selected and evaluated according to the Beers(2015),the frequency statistics method and multivariate Logistic regression was used to analyze the occurrence and related factors of potential inappropriate drug use in the elderly.Results: 157 cases(31.59%)there is at least one potential inappropriate drug use in elderly inpatients.Of these,127 cases(25.56%)used potentially inappropriate drugs related to drugs,48 cases(9.66%)used potentially inappropriate medication related to disease or state,114 cases(22.93%)used drugs with caution in elderly patients,43 cases(4.62%)improper use of drug interactions,13cases(2.62%)were treated with drugs based on renal function adjustment.According to the Logistic regression analysis,sex(P=0.004),age(P <0.01),the number of combined drugs(P=0.002),and renal function injury(P=0.025).There are 191 cases of PIM based on the catalogue of potential inappropriate drugs for the elderly in China,including 341 cases of A class warning drugs(Insulin and Clopidogrel have the highest frequency)and 71 cases of B class warning drugs(Central nervous system medication and Cardiovascular drugs have the higher frequency).Conclusion: 1.There isa potential inappropriate use of drugs in the elderly,mainly concentrated in the central nervous system drugs,anticholinergic drugs and NSAIDs drugs,which should be highly valued.2.Sex,age,combined use of drugs and renal function were associated with the occurrence of potentially inappropriate drug use in the elderly.3.The results of the two criteria / directory evaluations were basically similar in terms of drug types.However,the frequency of drug occurrence is different,because of the drug risk point / PIM reason is different between the two standards.4.Beers standard is of great value in evaluating potential inappropriate medication in elderly patients.Clinical pharmacists should draw lessons from and apply this standard to promote rational drug use in elderly patients and reduce the risk of medication in elderly patients.
