Objective:From the three aspects of chewing efficiency,grinding volume and satisfaction,whether the use of facial arches to record and transfer the patient's personalized hinge axis relationship during the production of full dentures is tested,which provides the clinical significance of facial arches in the process of full denture restoration Certain scientific basis.Methods:In this study,10 patients(aged 50-70 years)with missing upper and lower dentitions were selected.Each patient used two methods to make two different complete dentures,which were labeled as the control group and the experimental group.In the repair process,the second impression method was used to make the model.The rest jaw method and the facial shape observation method were used to record the vertical relationship of the patient and the touch temporal muscle method to determine the horizontal relationship.The vertical and horizontal relationship is transferred to the jaw frame.The clinical part of each case(mold taking,jaw position recording,initial wearing,etc.)is operated by the same clinician,and the mechanic part(rowing teeth,filling,polishing,etc.)is also the same.Completed by a mechanic to ensure comparability.The experimental group introduced the Gilbach facial arch record and transferred the patient's personalized hinge axis relationship.Based on the jaw position record,the Gilbach brace frame was used to make the restoration;the control group did not use the facial arch,and the average jaw frame was recorded based on the jaw position..The patient used a double-blind method to give one pair of two complete dentures for wearing,and changed to another pair for two weeks after two weeks.Record the adjustment time of each pair of dentures when they are first worn,and the number of re-diagnostics and adjustments during the wearing of the dentures within two weeks,which together serve as an evaluation index for the amount of adjustment of two complete dentures.The chewing efficiency and the satisfaction of the patients in five aspects of aesthetics,language expression,retention stability,chewing function,and comfort are collected.The relevant data are collected and analyzed.The research data is analyzed using SPSS 22.0 statistical software.Mean ± standard deviation)means paired t test is performed on the data subject to normality;Wilcoxon signed rank sum test for paired design is performed on the data that do not follow normality.Results:1.The adjustment amount of the control group is significantly higher than that of the experimental group(1)Initial wear adjustment time:the mean of the control group was 6.53 ± 1.51,the average of the experimental group was 5.00 ± 1.15,and the initial adjustment time was statistically significant between the control group and the experimental group(p 0.001),the initial adjustment time of the control group is longer than that of the experimental group;(2)in the number of grinding,the mean of traditional group is 5.00±1.33,the bow group,a surface mean of 2.90±1.37,the bow group,a number of grinding in traditional group and surface mean differences statistically significant(p<0.001),the traditional bow group,a group of times higher than that of surface grinding;2.The chewing efficiency of the control group is significantly lower than that of the experimental group(1)in terms of the efficiency of 30 mastication,the mean of the traditional group was 0.39 ± 0.12,and the mean of the arch group was 0.32 ± 0.11.The difference in the mean of the 30 mastication efficiency between the traditional group and the arch group was statistically significant(p<0.001),and the efficiency of the traditional group was lower than that of the arch group;(2)In terms of 20-second chewing efficiency,the average of the traditional group was 0.41±0.10,and the average of the face bow group was 0.36±0.09.The difference in the 20-second chewing efficiency between the traditional group and the face bow group was statistically significant(P<0.001),the 20-second chewing efficiency of the traditional group was lower than that of the facial bow group;3.Results of the satisfaction survey(1)In terms of aesthetic satisfaction,the average of the traditional group is 3.00±0.82,and the average of the face bow group is 3.40±0.84.There is no statistically significant difference in the average satisfaction of the traditional group and the face bow group p=0.223),it cannot be considered that the traditional group and the facial bow group are different in terms of aesthetic satisfaction;(2)In terms of language expression satisfaction,the average of the traditional group is 3.50±0.71,and the average of the face bow group is 3.80±0.63.The difference of the language expression satisfaction between the traditional group and the face bow group is not statistically significant(P=0.083),it can not be considered that the traditional group and the facial bow group are different in terms of language expression satisfaction;(3)In terms of satisfaction with retention stability,the average of the traditional group is 3.30 ± 0.48,and the average of the face arch group is 3.90±0.32.The difference in the stability stability of the retention group is statistically significant between the traditional group and the face arch group.Significance in learning(p=0.014),the satisfaction of retention stability of facial arch group is higher than traditional group;(4)In terms of chewing function satisfaction,the average of the traditional group is 3.10±0.57,and the average of the facial arch group is 3.80 ± 0.42.The difference in language function satisfaction between the traditional group and the facial arch group is statistically significant(P=0.008),the satisfaction of the che-wing function of the facial arch group was higher than that of the traditional group;(5)In terms of comfort satisfaction,the average of the traditional group is 3.30±0.82,and the average of the face arch group is 3.70±0.67.The difference in the average value of comfort satisfaction between the traditional group and the face arch group is statistically significant(p=0.046),the comfort satisfaction of the facial arch group is higher than that of the traditional groupConclusion:When determining the maxillary position of the patient during the production of complete dentures,introduce face arch records and transfer the patient's hinge axis relationship to the frame,which can make the complete dentures,the amount of grinding of the patient after wearing teeth is significantly reduced,the initial It has good adaptability when wearing,stable retention,and can obtain higher satisfaction in chewing function and comfort.
