Clinical Application Of Deep Brain Stimulation In The Treatment Of Minimally Conscious Patients

To explore whether DBS can promote the awakening of MCS patients,lay a foundation for the clinical treatment of MCS patients,and provide a safe and effective treatment idea.[method]The clinical data of 7 MCS patients who received DBS treatment in the Department of Minimally invasive Neurosurgery,Chenggong Hospital,the First Affiliated Hospital of Kunming Medical University from September 2017 to November 2019 were retrospectively analyzed.Statistical analysis software SPSS22.0 paired T test was used to compare the CRS-R scores of 7 patients with minimal consciousness before,1 month,3 months,6 months and 1 year after operation,and to observe the clinical efficacy of DBS in the treatment of MCS.[results]1.Influence of CM-PF-DBS on consciousness recovery of MCS patients.The CRS-R score(7.86±2.04)1 month after the operation of 7 patients was higher than that before the operation(5.57±2.23),and the difference was statistically significant(P=0.026<0.05).The CRS-R score(9.14±2.36)3 months after operation was higher than that(7.86±2.04)1 month after operation(P=0.049<0.05).Crs-r score(10.43±1.50)at 6 months postoperatively was higher than that at 3 months postoperatively(9.14±2.36),and the difference was statistically significant(P=0.022<0.05).There was no significant change in crs-r score(10.71±1.75)1 year after operation compared with(10.43±1.50)6 months after operation(P=0.172>0.05),indicating that cm-pf-dbs can be used as a feasible treatment idea for awakenings in patients with minimal consciousness,but the improvement of consciousness 6 months to 1 year after operation of cm-pf-dbs still needs further verification.2.Adverse reactions and complicationsIn this study,no obvious adverse reactions were observed in all 7 patients,such as intraoperative brain edema,secondary cerebral hemorrhage,secondary epilepsy,postoperative brain abscess,pulse generator shutdown or circuit failure,displacement of stimulating electrodes or extension wires,rejection reactions,etc.[conclusion]1.CM-Pf-DBS can be used as a feasible treatment idea for awakenings of patients with minimally consciousness state.2.The improvement of consciousness 6 months to 1 year after CM-PF-DBS still needs further verification.3.CM-Pf-dbs can improve the neurological function and consciousness level of patients with minimal state of consciousness,and has a certain effect of waking up.4.Electroencephalogram can objectively reflect the functional state of brain.
