Objective:The surgical treatment of craniopharyngioma is a difficult for neurosurgeons,which seriously affects the survival time and quality of life of patients.This paper summarizes the experience of our department in the treatment of craniopharyngioma with the frontolateral approach,and evaluates the surgical efficacy of the treatment of craniopharyngioma with the frontolateral approach,comparing with the traditional pterional approach.Methods:we collect postoperative pathology confirmed information for patients with craniopharyngioma in hospital of kunming medical university since 2017-06 to 2020-02,(32 cases of craniopharyngioma were saddle-on craniopharyngioma),17 cases by the frontolateral approach,15 cases by the traditional pterion approach,of the two approaches analysis of tumor resection and postoperative complications,and the frontolateral approach were discussed and summarized.Results:the frontolateral approach and pterion approach tumor all cut rates were 88.2%and 86.7%respectively,there was no statistically significant difference in the two groups(P>0.05),tumor of the cut,mainly associated with preoperative combined calcification,tumor size,preoperative combined calcification,tumor diameter greater than 4 cm craniopharyngioma cutting rate is low,all occurred on the two groups of patients with postoperative temporary diabetes insipidus,electrolyte disorders,fever,etc.,has nothing to do with two kinds of surgical approach,no statistical significance(P>0.05).However,the overall incidence of postoperative complications was low with the use of additional lateral approach,4 cases of subcutaneous effusion were found in the pterional approach,and 1 case of postoperative bleeding occurred in the operative area.Conclusion:For intracarapillary craniopharyngioma with diameter less than 4cm and no calcification,the frontolateral approach has the same effect on total craniopharyngioma as the pterional approach,but it has less trauma,fewer postoperative complications,and significantly improves the postoperative quality of life.Worthy of clinical promotion.
