Clinical Value Of Laparoscopic Median Approach CME In The Treatment Of Right Hemicolon Carcinoma

Objective:To investigate the clinical value of laparoscopic total mesocolectomy(LCME)for radical resection of right colon cancer,and to lay a theoretical foundation for the clinical application of laparoscopic radical resection of right colon cancer.Methods:according to the object of study inclusion criteria and exclusion criteria to select our hospital between January 2017-June 2019 treated 153 cases of right colon cancer patients as the research object,according to the divided into laparoscopic surgery way into the middle road whole mesocolon resection group(LCME)into the middle road whole mesocolon resection and laparotomy group(OCME group),which choose 91 cases of laparoscopic group,choose open group of 62 cases.The preoperative,intraoperative and postoperative clinical manifestations,clinicopathology and examination of the two groups were observed and compared.Results:the amount of intraoperative blood loss,incision length,postoperative exhaust time,number of postoperative additional analgesia cases,postoperative length of hospital stay,removal of all drainage tubes,and the time from postoperative to the first time when patients could get out of bed were significantly lower than those who chose the laparoscopic operation(P<0.05).The number of lymph nodes dissected intraoperatively by laparoscopy also has obvious advantages.However,there was no significant difference in operation time between the two methods(P>0.05).Postoperative serum c-reactive protein(CRP),interleukin 6(il-6)and tumor necrosis factor TNF-increased less in the laparoscopic group than in the open group(P<0.05).The postoperative values of CD3+,CD4+and CD4+/CD8+ decreased less in the laparoscopy group than in the laparotomy group(P<0.05),but the increase of CD8+in the laparotomy group was significantly higher than that in the laparoscopy group.The incidence of recent postoperative complications(pulmonary infection,incision infection,anastomotic leakage,ureteral injury,intestinal obstruction)in the laparoscopic group was significantly lower than that in the open group(P<0.05).Conclusion:laparoscopic CME into the middle road is the right half colon cancer radical treatment of right half colon cancer one big trend,small trauma,less intraoperative blood loss,postoperative body inflammatory reaction and immune function damage small,less postoperative complications,and many other advantages,is well worth more in-depth research and extensive application in clinical.
