[Objectives]We aimed to quantify the time to diagnosis among women with endometriosis in China and to analyze related factors affecting diagnostic delay.[Methods]A retrospective study was performed on 249 patients who underwent laparoscopic surgery and were diagnosed with ovarian endometriosis cyst by histopathology at the Department of Obstetrics and Gynecology,in Wuxi Maternal and Child Health Hospital between June 2014 and June 2018.[Results](1)In the 249 cases of endometriosis, the median delay in diagnosis was 14.0 years(0.5?42.0 years),and 78.3%(195/249)of the patients considered dysmenorrhea as a normal physiological phenomenon and did not see a doctor in time.(2)Single factor analysis shows that:The delay time of diagnosis of endometriosis patients with dysmenorrhea after menarche was significantly different from those with dysmenorrhea after menarche(16 vs5.5 years,P<0.01),the degree of dysmenorrhea found that changes in the patients with different diagnosis delay time have differences, dysmenorrhea gradually increase significantly shorter than in patients with dysmenorrhea unchanged and ease(P<0.05),the clinical diagnosis time>12 years significantly longer than in patients with clinical diagnosis time 12 years or less(P<0.01).In patients with severe dysmenorrhea diagnosis delay time(11.0 years)shorter than moderate pain(12.5years)and mild dysmenorrheal(14.5years),The delay time of diagnosis was slightly shorter in patients with deep infiltrating endometriosis, family history of endometriosis or patients with adenomyosis, but both were compared, the difference had no statistical significance(P>0.05);Multivariate analysis showed that the time of clinical diagnosis of dysmenorrhea and dysmenorrhea during menarche was an independent risk factor affecting the diagnosis delay(P<0.05).[Conclusions]The lack of attention to dysmenorrhea may be the main cause of delay in diagnosis of endometriosis. The main factors affecting the delay of diagnosis include the onset of dysmenorrhea at menarche, the change degree of dysmenorrhea symptoms, and the clinical diagnosis time of endometriosis.
