Objective:Cerebral ischemic stroke is a common cerebrovascular disease with high incidence rate,which seriously endangers people's physical and mental health.Inflammation and inflammatory factors play an important role in the pathophysiological process of ischemic stroke.MiRNA-21 may participate in pathophysiological processes of ischemic stroke and atherosclerosis by regulating inflammation,but the relationship between miRNA-21 and related inflammatory factors in plasma of patients with acute ischemic stroke is unclear.This experiment intends to detect the expression levels of pro-inflammatory factors(TNF-? and IL-6),anti-inflammatory factors(IL-4 and IL-10)and miRNA-21 in plasma of acute ischemic stroke patients,analyze the relationship between miRNA-21 and inflammatory factors,so as to provide molecular biological evidence for diagnosis and treatment of ischemic stroke.Methods:Ischemic stroke Patients(case group)and healthy subjects(control group)were selected as the research objects,and relevant clinical data were collected.Ischemic stroke patients were divided into mild stroke group(NIHSS<7 points)and severe stroke group(NIHSS?7 points)according to NIHSS score on admission.The fasting peripheral venous blood of subjects was collected,Real-time quantitative reverse transcription polymerase chain reaction(RT-qPCR)was used to examine the relative expression of miRNA-21 in plasma.Enzyme-linked immunosorbent assay(ELISA)was used to detect the expression levels of pro-inflammatory factors(TNF-? and IL-6)and anti-inflammatory factors(IL-4 and IL-10)in plasma.SPSS 23.0 software was used to analyze the data.Results:(1)The Plasma levels of TNF-? and IL-6 were significantly higher in the case group than in the control group(TNF-?:15.15±3.83 vs 12.57±1.54,P<0.001;IL-6:1.09±0.18 vs 0.44±0.10,P=0.009);The plasma levels of IL-4 and IL-10 had no statistical difference between the case group and the control group(P=0.211;P=0.902).Subgroup analysis of the case group showed that IL-6 level in severe stroke group(1.61±0.35)were significantly higher than that in mild stroke group(0.64±0.06),and the difference was statistically significant(P=0.012).However,the plasma expression levels of TNF-?,IL-4 and IL-10 had no difference between severe stroke group and mild stroke group(P>0.05).(2)The relative expression of plasma miRNA-21 in the case group(1.69±0.28)was significantly higher than that in the control group(1.08±0.17),and the difference was statistically significant(P=0.042),but there was no statistical difference(P=0.889)of miRNA-21 between mild stroke group(1.65±0.28)and the severe stroke group(1.73±0.53).(3)Correlation analysis between miRNA-21 and inflammatory factors showed that miRNA-21 was positively correlated with pro-inflammatory factors TNF-?(r=0.466,P=0.002).No correlation between miRNA-21 and other inflammatory factors was found.Conclusions:(1)The acute stage of ischemic stroke is a process of acute inflammatory reaction.The expression of relevant pro-inflammatory factors(TNF-? and IL-6)are significantly increased,Anti-inflammatory factors(IL-4 and IL-10)have no obvious changes.Moreover,IL-6 is related to the degree of neurological impairment and can be used as one of biological indicators of the severity of ischemic stroke.(2)Circulating miRNA-21 is positively correlated with pro-inflammatory factor TNF-?,which may participate in the pathophysiological process of acute ischemic stroke through positive regulating of TNF-?,and is expected to become one of the diagnostic indicators and therapeutic target genes of ischemic stroke.
