ObjectiveThe purpose of this study was to retrospectively analyze the clinical effect of totally thoracoscopic cardiac surgery for repairing atrial septal defect.MethodsThe clinical data of 104 patients who underwent totally thoracoscopic cardiac surgery for atrial septal defect repair in the Department of Cardiac surgery of Qilu Hospital of Shandong University from December 2017 to December 2019 were collected.The patients were 2-76 years old and weighed between 15-126 kg.The secondary atrial septal defect was diagnosed by clinical color Doppler echocardiography,including 47 cases of cardiac arrest repair under totally thoracoscopic cardiac surgery,57 cases of beating-heart repair.Through the collection of admission records,auxiliary examination reports,operation records,surgical handover orders,cardiopulmonary bypass records and body temperature records of the two groups,statistical software was used to calculate and analyze the data.ResultsThe preoperative basic data of the two groups were compared,and it was found that there was no significant difference in age,sex,body mass,defect diameter and cardiac function grade between the two groups(P<0.05).There was no operative death or conversion to thoracotomy in this study.The total operation time,cardiopulmonary bypass time and ventilator assisted ventilation time in the cardioplegia group were significantly longer than those in the cardioplegia group(P<0.05),the ICU stay time and postoperative hospital stay in the cardioplegia group were significantly shorter than those in the cardioplegia group(P<0.05),and the thoracic drainage within 24 hours in the cardioplegia group was significantly less than that in the cardioplegia group.There were no serious perioperative complications in both groups,and there was no shunt at atrial level by color Doppler ultrasound before dischargeConclusionsTotally thoracoscopic for closure of atrial septal defect is safe and feasible.Totally thoracoscopic for closure of atrial septal defect on beating heart requires higher technical requirements than that of off-pump heart.Totally thoracoscopic for closure of atrial septal defect has more advantages.It can be used as an alternative to conventional median thoracotomy for atrial septal defect.
