Objective:To explore the main influencing factors and correlation analysis of colorectal adenoma in middle-aged and elderly patients with type 2 diabetes. Methods:The study subjects selected 316 patients with type 2 diabetes who underwent a complete colonoscopy in the Department of Gastroenterology in our hospital from December 2015 to December 2018.A retrospective study was conducted, and all patients were divided into adenoma groups according to whether there was an adenoma or not.And non-adenomas group, including 157 cases in the adenoma group and 159 cases in the non-adenomas group. The age, sex, nationality, characteristics of many colorectal polyps, and blood routine were compared between the two groups.Multivariate Logistic regression analysis was used to determine independence. For risk factors, use Spearman correlation to analyze the correlation between the two. Results: Comparing the baseline data and laboratory indicators of the adenoma group and non-adenomas group,it was found that all indicators of the two groups were not statistically significant,P>0.05,Comparing the clinical characteristics of the two groups of patients,it was found that the adenoma group The number of patients with polyps greater than 0.5 cm,the number of patients with polyps greater than 3,and the number of patients with adenomas were significantly higher than those in the non-adenoma group,P <0.05,and the differences were statistically significant.By comparing the course of diabetes and hematological data of patients in the adenoma group and the non-adenomas group,it was found that there was a statistical difference between the two groups of patients with diabetes,P<0.05,and the difference was statistically significant. However, there was no significant difference in hematological data between the two groups,P>0.05,and the difference was not statistically significant. The univariate analysis found that the factors affecting colorectal adenomas in patients withtype 2 diabetes include the duration of diabetes,Hb A1 c,and insulin, and multivariate logistic regression analysis found that the duration of diabetes,Hb A1 c,and insulin caused type 2 diabetes.According to Spearman correlation analysis, the occurrence of colon adenomas in patients with type 2 diabetes was related to the gender and duration of diabetes, which was independently positively correlated with the duration of diabetes,P<0.05,the difference was statistically significant. Conclusion: Colorectal adenomas are more harmful in patients with type 2 diabetes.Multivariate analysis revealed that the duration of diabetes,Hb A1 c,and use of insulin may be the main risk factors for colorectal adenomas in patients with type 2 diabetes. The incidence of polyps is independently and positively correlated, suggesting that patients with the above three factor should be perfected colonoscopy in the clinic, which can be used as a new test index for detecting colorectal adenomas, and it has good prevention and prognosis of colorectal adenomas. Its clinical value is worth promoting.
