Objectives:It is significant to know the shape and size of sella turcica for identifying and managing pituitary diseases and determining orthodontic treatment plans. The objectives of this study were:1.To study and measure the size of sella turcica on the lateral cephalogram of HE patients with different skeletal malocclusion, and record the shape and size of the sella turcica and the incidence of sella bridge;2.To analyze the connections between the size and shape of sella turcica, the incidence of sella bridge and the specific measurement values of different craniofacial structures. Methods:Lateral cephalogram was photographed for 332 patients, aged 15 to 30 and with an average of 22.1,who saw a doctor in the department of orthodontics of the first affiliated hospital of Dalian medical university from 2018 to 2019.The X-ray films were measured, wherein the size of sella turcica, the incidence of sella turcica bridging and the maxillofacial bone tissue were measured. The statistical analysis was carried out for the obtained data by using SPSS 23.0 statistical software.,and the connections between the size of sella turcica and the class I,II and III skeletal patterns were analyzed,as well as the connections between the size of sella turcica and the measurement values of the maxillofacial bone,so that the incidence of sella bridge between different skeletal patterns was obtained, and the connections between the incidence of sella bridge between maxillofacial region, especially the maxilla and mandible, was studied.Results:1.The depth of sella turcica in the patients with skeletal pattern II was greater than that in the patients with skeletal patterns I and III,and the result was statistically significant(P<0.01).The area of sella turcica in skeletal pattern II patients was greater than that in the skeletal patterns I and III,and the result was statistically significant(P<0.05).The length of sella turcica in the skeletal pattern III was greater than that in the skeletal patterns I and II,but the result was not statistically significant(P>0.05).2.(1)The depth of sella turcica was positively related to 11 measurement values, such as SNB,ANB,NA-PA,YAix,anterior and posterior height, maxillary length, mandibular angle, mandibular height, mandibular body length and PP-FH,while the depth of sella turcica was negatively related to SNB and mandibular angle.(2)The length of sella turcica was negatively related to YAix.(3)A significant negative correlation was showed between the ratio of diameter of sella turcica and face height and the mandibular angle;and a positive correlation was showed between the diameter of sella turcica, the SN-PP,the anterior height and the mandibular length.(4)A negative correlation was showed between the area of sella turcica and the mandibular angle;and the area of sella turcica was positively related to the SN-PP,the mandibular height, the mandibular length, the anterior height and the maxillary length.3.For the incidence of sella bridge of different skeletal patterns:the patients with the normal sellar shape was 61.7%;the incidence of sella bridge was 38.3%,wherein 96 cases(75.6%)were partial sella bridge and 31 cases(24.4%)were complete sella bridge. The incidence of sella bridge of different skeletal patterns was 61.9%for class III,34.5%for class II and 29.8%for class I.The incidence of sella bridge was 44.7%for male and 35.6%for female, and there was not statistically significant.4.The statistical analysis showed that significant statistical differences in SNB,ANB,NA-PA,AO-BO and the mandibular length existed between the patients with the normal sella turcica and the patients with sella bridge. No statistical significance was showed in the maxillofacial measurement between partial and complete sella turcica patients.Conclusions:1.The size of sella turcica was related to the skeletal patterns. The depth of sella turcica in the skeletal pattern II was greater than that in the skeletal patterns I and III;the length of sella turcica in the skeletal pattern III was greater than that in the skeletal patterns I and II.The scope of the skeletal pattern I patient’s sella area was 0.67±0.14 cm~2.2.The size of sella turcica was related to both maxilla and mandible, but the correlation with the growth direction and size of mandible was better. The growth of maxilla and mandible could be predicted clinically.3.The incidence of sella bridge was38.3%,wherein the incidence of sella bridge in male was higher than that in female;the incidence of sella bridge in skeletal pattern III was higher than that in skeletal pattern I and II.4.A trend of class III malocclusion in patients with sella bridge was showed by the comparison of the measurement values of the patients with sella bridge and those without sella bridge.
