Objective : To explore the value of the changes of NLRP2,ENO1 and inflammatory factors in the course of sepsis in evaluating the condition and prognosis of patients with sepsis.To provide a new basis for the diagnosis and treatment of sepsis.Methods : According to the The Third Edition of International Consensus Definition of Sepsis and Septic Shock,the patients who diagnosed with sepsis admitted to emergency ICU(EICU)in the first affiliated Hospital of Dalian Medical University from March 2019 to February 2020 were selected and divided into the group of sepsis and septic shock according to the severity of the patients.There were 24 patients in the group of sepsis and 19 patients in the other.At the same time,10 patients with non-sepsis were selected as the control group.According to prognosis after 28 days,the patients were divided into survival group(n = 23)and death group(n = 20).Peripheral venous blood within 48 hours after admission was sent to the laboratory department in the hospital to detect the inflammatory markers such as interleukin-6(IL-6),interleukin-10(IL-10),procalcitonin(PCT),C-reactive protein(CRP),nutritional index albumin(ALB)and metabolic index lactate(Lac),and 10 ml of peripheral venous blood of the serum nucleotide binding and oligomerization domain like receptor 2(NLRP 2)and ? enolase(ENO 1)were detected by enzyme linked immunosorbent assay((ELISA))in two hours.Then other important clinical indexes and vital signs clinical data were recorded,and the sequential organ failure score((SOFA))and acute physiology and chronic health score ?(APACHE ? score)were also scored.The changes of the above biomarkers between groups were analyzed and compared,and the change levels of biomarkers in different pre-groups were evaluated.Results:1.The level of NLRP 2 in sepsis group and sepsis shock group was higher than that in non-sepsis group,and the level in sepsis shock group was higher than that in sepsis group,and the difference was statistically significant.2.The levels of CRP,PCT,IL-10,Lac and CRP/ALB in septic shock group were higher than those in septic group,while the levels of IL-6 and ALB in septic shock group were lower than those in septic group,and the difference was statistically significant.3.NLRP 2,CRP,PCT,IL-6,IL-10,CRP/ALB were positively correlated with SOFA score,and ENO 1,ALB was negatively correlated with SOFA score.4.NLRP 2 and ENO 1 were respectively correlated with PCT,CRP,IL-6 and IL-10.NLRP 2 was positively correlated with PCT,CRP,IL-6 and IL-10,and ENO-1was positively correlated with IL-6 and IL-10,and the results were significant.Conclusion:1.Combined detection of NLRP-2,CRP,PCT,IL-6,IL-10,Lac and other biomarkers,the combination of pro-inflammatory factors and anti-inflammatory factors can evaluate the developing direction of patients with sepsis and provide corresponding basis for the treatment of sepsis.2.The increase of serum CRP,the decrease of ALB and the high level of CRP/ALB in patients with sepsis can indicate a poor prognosis.CRP/ALB is a combined parameter,which is less affected by other factors,so it can provide a more valuable basis for evaluating the prognosis of patients.3.Combined detection of a variety of biomarkers to evaluate the condition and prognosis of patients with sepsis is better than a single biomarker.
