Objective: In recent years,it has been found that vitamin D3 plays an important role not only in the regulation of calcium and phosphorus,but also in the inhibition of inflammation and immune response,cardio cerebral vascular protection and renal protection.Vitamin D deficiency is common in patients with chronic kidney disease,especially in patients with primary nephrotic syndrome.This article will explore the relationship between vitamin D3 and idiopathic membranous nephropathy,and explore the role of vitamin D3 supplementation in the treatment of idiopathic membranous nephropathy.Methods: From June 2018 to June 2019,the patients with primary nephrotic syndrome who were hospitalized in the Department of Nephrology,the first people's Hospital of Jining City were identified as membranous nephropathy by renal biopsy 66 patients with membranous nephropathy were included in the study.The quantitative levels of sex,age,vitamin D3,albumin,cholesterol,triglyceride,high-density lipoprotein,low-density lipoprotein,fibrinogen,calcium ion and 24-hour urine protein were recorded.66 healthy people in physical examination center were collected and their sex,age and vitamin D3 level were recorded.To analyze the difference of vitamin D3 level between the patients with membranous nephropathy and the healthy group,and to analyze the correlation between the level of vitamin D3 in the patients with membranous nephropathy and the above research indicators 66 patients with membranous nephropathy were randomly divided into control group and experimental group.The control group was treated with hormone(1mg / kg.d),cyclophosphamide(0.8g / time,once a month),low-molecular-weight heparin,simvastatin and so on.The experimental group was treated with 0.25ug/d of calcitriol Capsule on the basis of the same treatment scheme as the control group.The levels of vitamin D3,triglyceride,high-density lipoprotein,cholesterol,low-density lipoprotein,albumin,calcium ion and 24-hour urine protein were recorded.The changes of indexes in the control group and the experimental group before and after treatment were analyzed.SPSS 19.0 software was used for data statistical analysis,P < 0.05 was statistically significant.Results:1.In 66 patients with idiopathic membranous nephropathy,the serious deficiency rate of vitamin D3 was 10.6%,the deficiency rate was 78.8%,and the deficiency rate was 10.6%;in 66 healthy people,the deficiency rate of vitamin D3 was 7.6%,no serious deficiency and deficiency of vitamin D3 were found.2.Serum vitamin D3 was negatively correlated with triglyceride,cholesterol,low-density lipoprotein,fibrinogen and 24-hour proteinuria(P<0.01),and positively correlated with high-density lipoprotein,calcium ion and albumin(P < 0.01).3?Vitamin D3 was negatively correlated with triglyceride,24-hour albuminuria,cholesterol,fibrinogen and low-density lipoprotein(P < 0.01),and positively correlated with high-density lipoprotein,calciumion and albumin(P < 0.01).4?There was no significant difference between the control group and the experimental group in gender,age,vitamin D3,serum albumin,cholesterol,triglyceride,low-density lipoprotein,fibrinogen,calciumion and 24-hour urine protein levels before treatment(P > 0.05).5?After treatment,proteinuria,cholesterol,triglyceride,fibrinogen and low-density lipoprotein in the control group and the experimental group decreased,while albumin and calcium ion increased(P < 0.05).6?The decrease of urine protein,cholesterol,fibrinogen and LDL in the experimental group was better than that in the control group(P < 0.05),and the increase of albumin and calcium in the experimental group was better than that in the control group(P < 0.05).There was no significant difference in the decrease of triglyceride between the experimental group and the control group(P>0.05).Conclusion:1.Vitamin D3 deficiency is prevalent in patients with idiopathic membranous nephropathy.2.Vitamin D3 in idiopathic membranous nephropathy was negatively correlated with cholesterol,triglyceride,low-density lipoprotein,fibrinogen,24-h proteinuria,and positively correlated with albumin,high-density lipoprotein and calcium ions.3.Vitamin D3 supplementation can improve serum albumin,24-hour urine protein,fibrinogen,low-density lipoprotein and calcium in patients with idiopathic membranous nephropathy.
