Objective:To explore heart failure with preserved ejection fraction(HFPEF)and atrial arrhythmia(AR).Mainly from the following aspects to study.To study the prevalence of HFPEF in patients with atrial arrhythmia,the correlation between the complexity of atrial arrhythmia and the retention of diastolic function of heart failure by ejection fraction,and the relationship between the risk factors.So as to screen the risk factors in the early stage,further explore the diagnosis and treatment basis of heart failure with preserved ejection fraction,and make early intervention according to the results,so as to effectively reduce the heart strength with preserved ejection fraction Patients with failure eventually progress to heart failure probability with reduced ejection fraction,thereby reducing hospitalization rate and mortality.Methods:1.Retrospective analysis of 152 patients with normal ejection fraction from September 1,2017 to December 31,2019 in outpatient and cardiology department of the People's Hospital of Liaoning Province.After screening cases according to inclusion and exclusion criteria,clinical case data of 152 patients were included.According to whether it is combined with atrial arrhythmia,118 patients were divided into atrial arrhythmia group(the atrial arrhythmia was divided into 1-6 grades according to kleiger classification),and 34 patients in non atrial arrhythmia group.2.The patient's case information was recorded in detail,including gender,age,Symptoms existing this time,heart failure,hypertension,type 2 diabetes mellitus with or without preserved ejection fraction,serological indexes including NT-pro BNP,related ultrasound indexes including E / E ',e'peak(e'),The ratio of early and late diastolic peak velocity(E / A),Left ventricular ejection fraction(LVEF)value,left atrial diameter(LAD),Other indicators include body mass index(BMI),and dynamic ECG results.The diastolic function of HFPEF was graded under the guidance of echocardiography.3.Spss23.0 statistical software was used to analyze the data and compare the differences among groups.The test indexes obtained by each group are described by the standard deviation of the mean(X ąS),and single factor variance analysis was used for comparison between groups.Chi square test was used to determine whether there was any difference in heart failure rate reserved by ejection fraction between atrial arrhythmia group and non atrial arrhythmia group.Spearman correlation analysis was used to study the classification of atrial arrhythmia and HFPEF diastolic function.Binary logistic regression analysis was used to study the risk of disease,and the difference was statistically significant when p < 0.05.Result:1.The analysis of basic data between groups of atrial arrhythmia showed that:there were significant differences among groups of E / E ',E/A,LAD and LVEF(P <0.05);there were no significant differences among groups of age,gender,e'peak,BMI,type 2 diabetes mellitus and hypertension(P>0.05).2.Compared with the heart failure rate of the normal control group,the heart failure rate of the atrial arrhythmia group was higher(P < 0.05).3.There was a significant positive correlation between the grading of atrial arrhythmia and the grading of diastolic function of HFPEF.4.Binary logistic regression analysis showed that the size of left atrium diameter was the risk factor of HFPEF,(OR=1.402,P=0.001).5.There was no significant difference in age,e'peak and BMI among the groups(P>0.05),but there was significant difference in E/E ',E/A,left atrial size,NT pro BNP and EF among the groups(P<0.05).The number of patients with left atrial size,NT pro BNP,LVEF,E / E 'and hypertension in the group of atrial arrhythmia with HFPEFwas higher than that in the group without HFPEF and the control group(P < 0.05);the number of patients with E/A,left atrial size,LVEF and type 2 diabetes in the group of atrial arrhythmia without HFPEF was higher than that in the control group(P<0.05).Conclusion:1.The prevalence of heart failure in patients with atrial arrhythmia was higher than that in people without atrial arrhythmia.2.The diastolic function grade of heart failure preserved by ejection fraction is related to the degree of atrial arrhythmia(complexity).The higher the diastolic function grade of HFPEF is,the higher the degree of atrial arrhythmia is.There is a significant positive correlation between the two.3.The increase of left atrial diameter is a risk factor of HFPEF in patients with atrial arrhythmia.4.The number of patients with left atrial size,NT pro BNP,LVEF,E / E 'and incidence rate of hypertension in the group of atrial arrhythmia with HFPEF was higher than that in the group without HFPEF and the normal group.(No atrial arrhythmia and no HFPEF)...
