Objective: To investigate the feasibility and necessity of cervical cancer screening in the early postpartum period by using hpv dna tct and hpv e6/e7 mrna tct methods to analyze the results of cervical cancer screening and compare the data with the results of the general population.Methods: A total of 2035 pregnant women who met the conditions ~ cervical cancer screening and agreed to perform cervical cancer screening were selected for 42 days ~3 months postpartum reexamination in the postpartum outpatient department of xxx Maternal and Child Health Hospital from October 2017 to August 2019.HPV DNA test / HPV E6/E7 mRNA test and TCT test were randomly performed.the subjects were divided into HPV DNA +TCT groups and HPV E6/E7 mRNA +TCT groups according to the different detection methods selected.Record maternal general data and HPV DNA test / HPV E6/E7 mRNA test and TCT test results,and follow-up diagnosis and outcome data for statistical analysis.Results: 1,general data: there was no significant difference between the two groups,such as age,number of births,mode of delivery,screening for cervical cancer prevention before pregnancy(p>0.05).2,Of the 2035 subjects studied,1104 were HPV DNA+TCT,accounting for 54.25%(1104/2035);931 were HPV E6/E7 mRNA +TCT,accounting for 45.75%(931/2035).The positive rate of hpv dna was 17.12%(189/1104),the positive rate of HPV E6/E7 mRNA was 12.46%(116/931),and the positive rate of hpv was 14.99%(305/2035),which was lower than that of the general population.TCT≥ASC-US 298 cases,the abnormal detection rate was 14.64%(298/2035),of which ASC-US 194 cases,the detection rate was 9.53%(194/2035),significantly higher than the general population;ASC-H 11 cases,the detection rate was 0.54%(11/2035),comparable to the general population;LSIL 57 cases,the detection rate was 2.80%(57/2035),comparable to the general population;HSIL 36 cases,the detection rate was 1.77%(36/2035),comparable to the general population.3,According to the screening results,135 colposcopy patients accounted for 6.63% of the total cases(135/2035),slightly higher than the general population4,The pathological biopsy was taken under colposcopy in 96 cases,and the biopsy accounted for 4.72%(96/2035)of the total number of cases,and the biopsy rate was higher than that after cervical cancer prevention screening in the general population.5.Results of pathological biopsy in 96 cases :30 cases of cervicitis / normal,LSIL 35 cases,HSIL 31 cases,the detection rate of SIL was 3.24%(66/2035),which was lower than the detection rate of cervical cancer screening biopsy in ordinary people.6.The pathological biopsy results were ≥LSIL positive,and the specificity,positive predictive value and negative predictive value of HPV E6/E7 mRNA detection method were higher than those of HPV DNA detection method(p<0.05).7.The 269 TCT≥ASC-US(ASC-US 194;ASC-H 9;LSIL 55;and HSIL 11)subjects were followed up(6 months).Results TCT 269 subjects :194 ASC-US cases,190 cases,4 cases,ASC-US;9 cases,3 cases,ASC-H,6 cases.There were 55 LSIL cases HSIL,19 2 cases were still LSIL,8 cases were reversed to ASC-US,26 cases of cytology.There were still HSIL,2 cases of LSIL in 11 cases,Among 269 cases,97.9 per cent(190/194)of ASC-US cases had a cytological conversion rate of 82.53 per cent(222/269).Conclusion: 1,Cervical cancer screening within 3 months postpartum is timely and necessary.2,TCT ASC-US detection rate in 3 months postpartum was higher than that in the general population.3,The detection rate of TCT ASC-H 、LSIL and HSIL within 3 months postpartum is comparable to that of the general population.4,During 3 months postpartum hpv the positive rate was lower than that of the general population.5,The rate of colposcopy examination within 3 months postpartum is slightly higher than that of the general population,and the rate of colposcopy biopsy is higher than that of the general population after cervical cancer prevention screening,while the detection rate sil biopsy results is lower than that cin common population cervical cancer prevention screening biopsy results.6,The positive rate HPV E6/E7 m RNA cervical cancer screening is lower than that of HPV DNA,but it has higher specificity,positive predictive value and negative predictive value.
