Objective Large vestibular aqueduct syndrome(LVAS)is a congenital inner ear disease with progressive and fluctuating hearing loss as the main manifestation.It often causes sudden hearing loss due to slight head injuries and cold.Effective and standardized preventive and treatment methods have not been established.The hearing loss of most LVAS patients decreases year by year until they are deaf,which seriously affects their daily lives and hearing and speech rehabilitation.As a result,a large proportion of patients eventually need cochlear implantation.This article studies the effective methods of preventing and treating sudden hearing loss in LVAS with Chinese medicine,by observing the difference in efficacy between different methods of hormone therapy and Chinese medicine combined with western therapy,and compares the recurrence of hearing fluctuations before and after hearing management in patients with LVAS.Explore effective and safe preventive methods and hearing management strategies for LVAS to control or delay LVAS hearing loss from getting worse.Methods A total of 539 LVAS patients who attended the Deaf Rehabilitation Institute of Zhejiang Chinese Medicine University from February 2018 to March 2019 were selected as the research subjects.Among them,137 LVAS patients who had sudden hearing loss were selected by object of study.It was divided into an experimental group of 62 cases with dialectical treatment of traditional Chinese medicine;26 cases of a control group were treated with methylprednisolone,mecobalamine,and Extract of Ginkgo Biloba Leaves Drops;another group of 49 patients were treated with dialectical treatment of traditional Chinese medicine combined with methylprednisolone,mecobalamine,and Extract of Ginkgo Biloba Leaves Drops.All three groups of LVAS patients were treated for 21 days.Observe the outcomes of air conduction thresholds at different frequencies in the three groups of patients and side effect,and explore the effectiveness and safety of the experimental group in treating sudden hearing loss in LVAS.For 539 LVAS patients(including 137 LVAS patients who were stabilized after emergency rescue),three dimensional hearing management strategy was developed to maintain hearing stability:by establishing full clinical case data for LVAS patients and uploading them to the corresponding hearing central management platform,regularly checks the hearing of patients,monitors the hearing dynamics in real time,and achieves digitized of hearing management.It provides online assistance services for LVAS patients and their families,unified Internet management,and establishes emergency channels for 12 hours a day.Sudden Hearing loss is dealt with in a timely manner,so that hearing management can be networked;treatment based on syndrome differentiation,prevention by traditional Chinese medicine,mainly based on tonifying the kidney and strengthening the spleen,improving spleen and water,combined with different clinical manifestations and imaging characteristics of patients to add and subtract,prevent and control the progressive decline of hearing to achieve personalized hearing management.The changes in hearing thresholds at different frequencies before and after hearing management and the number of episodes were recorded,and the recurrence rate was counted to explore whether the three dimensional hearing management can effectively control and delay LVAS hearing loss from getting wrose.Results 1.After treatment,the hearing thresholds of LVAS patients in the experimental group were significantly better than before treatment(P<0.01).Among the patients in the experimental group,there were no obvious side effect.There were 5 cases of full moon face and 2 cases of skin itch in control group 1;3 cases of full moon face and 2 cases of irritability in control group 2.2.After 539 cases of LVAS patients passed the two-year three dimensional hearing management,the recurrence rate decreased significantly from 99.63%before hearing management to 31.59%after hearing management.The number of 0 relapses increased from 2 cases before hearing management to 366 cases;the average number of recurrences 1 decreased from 471 cases before hearing management to 148 cases;the average number of relapses 2 was from 50 cases before hearing management to 14 cases;the number of patients with an average recurrence greater than 2 decreased from 12 to 7 after hearing management,and the number of recurrences decreased significantly(?2=550.15,P<0.05).Conclusion 1.Dialectical treatment of traditional Chinese medicine has a significant effect on LVAS hearing loss,and the side effect are less than traditional hormone therapy.2.Three dimensional hearing management for LVAS patients can effectively prevent and control progressive hearing loss,retain residual hearing,and improve the quality of life and rehabilitation effect.
