Abstract/Summary:
Objective:To explore the influencing factors of neoadjuvant chemotherapy for breast cancer and guide clinical individualized treatment.Methods:To collect patients who were diagnosed with invasive breast cancer and received neoadjuvant chemotherapy from January 2017 to June 2019 in a hospital,and their clinical data and immunohistochemistry were analyzed retrospectively.SPSS software was used for statistical analysis.The clinical efficacy assessment and pathological efficacy assessment were performed using Kappa consistency test.The single factor analysis of the correlation between each influencing factor and the efficacy of chemotherapy was performed using χ2 test.Multivariate analysis was performed using binary logistic regression analysis.Result:In 219 patients,the effective rate of chemotherapy was 59.8%,and the PCR rate was 12.8%.The results of single analysis showed that there were statistically significant differences between the breast cancer patients with differenr molecular types,ER expression levels,B-ultrasonic blood flow signals and fibrinogen changes after neoadjuvant chemotherapy in the clinical efficiency of chemotherapy and the rate of PCR(P < 0.05).Chemotherapy dfficacy of triple-negative breast cancer is better than Luminal type.Patient’chemotherapy efficacy in the ER-negative group are higher than those in the ER-positive group.Patients with reduced B-ultrasound blood flow and fibrinogen after neoadjuvant chemotherapy are better chemotherapy-efficacy and PCR rate than those in stable or elevated group.There were statisatistical differences in KI-67 expression level,tumor size,and PCR rate(P <0.05);In terms of PCR rate,KI-67 ≥ 30% type was higher than KI-67<30%,and group of tumor diameter≤3cm was better than that of tumor diameter > 3cm.Mutivariate analysis results showed that different molecular types,ER expression,,and changes in fibrinogen and B-ultrasound blood flow signals after neoadjuvant chemotherapy were independent factors affecting the effectiveness of neoadjuvant chemotherapy(P<0.05).Conclusion:1.Changes in fibrinogen levels and changes in blood flow signals can be used to evaluate the efficacy of neoadjuvant chemotherapy for breast cancer;2.Breast cancer molecular typing,ER expression level,KI-67 expression level,tumor size can be used as predictors of the efficacy of neoadjuvant chemotherapy.
