BackgroundMigraine is a clinically common primary headache characterized by recurrent of moderate to severe throbbing headaches.It is common in young and middle-aged,and may have auras such as visual or sensory disturbances.Migraine has a high incidence all over the world,which brings great pain to the patients and heavy burden on the society.The pathogenesis of migraine has not been fully explored and there are many hypotheses.In recent years,the studies on the relationship between homocysteine(Hcy)and migraine has attracted attention.Further exploration has found that the gene polymorphism affecting the plasma homocysteine metabolism may be involved in the occurrence and development of migraine and other pathophysiological processes.The research on the association between methylenetetrahydrofolate reductase(MTHFR)gene polymorphism and migraine has become a hot topic.There are many polymorphisms of MTHFR gene,especially the two sites of C677 T and A1298 C are most concerned.However,the reported results of Hcy and MTHFR gene polymorphisms and migraine are still controversial at home and abroad.ObjectiveTo investigate the association between the methylenetetrahydrofolate reductase C677 T and A1298 C gene polymorphisms and plasma homocysteine with the risk of migraine in han population in northeast China,and to study the effect of MTHFR gene polymorphism on plasma Hcy level in migraine patients.To explore the pathogenesis ofmigraine from the genetically,and provide more evidence for the development of migraine treatment plans.MethodsPatients with migraine inpatients and outpatients in department of neurology,Liao ning Hospital from March 2018 to December 2019 were collected as the migraine group,and healthy patients who were collected during the same period were used as the control group.All the Han ethnic group in northeast China has no blood relationship with each other.Baseline data and clinical characteristics of migraine were collected from the two groups.Plasma Hcy levels were detected by direct chemiluminescence method.Gene polymorphisms of the MTHFR C677 T and A1298 C were detected by Sanger sequencing.Statistical analysis of the genotypes and alleles frequencies distribution and the differences in plasma Hcy levels in migraine and control groups,and analysis of the relationship between genotypes and plasma Hcy levels in migraine groups.SPSS22.0software was used for statistical analysis.Chi-square test or Fisher’s exact probability method were used for comparison of genotypes and alleles between groups,and non-parametric tests was used for comparison of plasma Hcy group and their relationship with genotypes.The control group used the chi-square test for the Hardy-Weinberg genetic balance.Using bilateral test,P < 0.05 was considered statistically significant.Results1.A total of 141 subjects were included in this study,81 cases of migraine,including 12 cases of migraine with aura(MA),69 cases of migraine without aura(MO);there were 31 males and 50 females,aged 19-65 years.There were 60 patients in the control group,including 23 males and 37 females,aged 29-61 years.There was no statistical difference in age,gender,and occupation between the two groups(P=0.328;P=0.994;P=0.919).2.The plasma Hcy level of the migraine group was higher than that of the control group,and the difference was statistically significant(P=0.023).3.There was no statistically significant difference in the genotype frequencydistribution of MTHFR C677 T between the migraine group and the control group(P=0.098),and the difference in the T allele frequency was statistically significant(P =0.033);Compared with the control group,the MO and MA subgroups of migraine group at C677 T showed no significant difference in the distribution frequencies of MA and MO genotypes and alleles(P=0.303,P=0.101;P=0.167,P=0.057).There was no statistically significant difference in genotype and C allele frequencies distribution between the migraine group and the control group at A1298C(P=0.628,P=0.672),and there was no statistically significant difference in further analysis of MA and MO subgroups(P=1.000,P=1.000;P=0.766,P=0.630).4.The plasma Hcy levels of different genotypes at the C677 T locus in the migraine group were analyzed,the plasma Hcy levels corresponding to TT,CT and CC genotypes were different,and the differences were statistically significant(P=0.001).Further comparison of plasma Hcy to the three genotypes,patients with TT genotype migraine were higher than CT genotype(P = 0.005)and higher than CC genotype(P = 0.006).The Hcy level of CT genotype was slightly higher than that of CC genotype,but there was no statistical difference(P=1.000).There was no significant difference in plasma Hcy of AA and AC genotypes at A1298 C in migraine group(P=0.196).Conclusion:1.The gene polymorphism of MTHFR C677 T is related to the incidence of migraine in Han population in northeast China,and the T mutation may increase the susceptibility to migraine.2.The gene polymorphism of MTHFR A1298 C is not related to the incidence of migraine in the Han population in northeast China.3.Increased plasma Hcy level may be a risk factor for migraine.4.Plasma Hcy level in migraine patients is related to gene polymorphism of MTHFR C677 T,and TT homozygous mutation may lead to increased plasma Hcy level.5.The AA and AC genotypes at the MTHFR A1298 C were independent of the plasma Hcy level in migraine patients.
