Background and Aim:Growth hormone deficiency(GHD)is the main reason why children are short.People gradually pay attention to a series of problems brought about by GHD with the improvement of medical standards and health awareness.At present,children with GHD are mainly using recombinant human growth hormone(rhGH)as an exogenous compensatory alternative treatment.The effectiveness of rhGH treatment has been applied on a larger scale in the clinic.Clinicians try to set up different treatment schemes to study the curative effect of rhGH in promoting growth,formulate better individualized treatments,and expect to achieve satisfactory treatment results.The purpose of this project is to observe and contrast whether there are differences in the curative effect of growth indexes of children with GHD under two different treatment schemes in the intermittent treatment group and the continuous treatment group.Research Object and Method:In this study,GHD patients who had been treated with rhGH for more than3 years were selected as the research object.Screening of patients with GHD confirmed in The Second Affiliated Hospital of Dalian Medical University as intermittent treatment group who continuously receiving rhGH treatment in the first year,height growth was not significant due to seasonal growth and recombinant human growth hormone intervention(<0.4cm/month),and choose to discontinue GHD for 2 to 3 months when insulin-like growth factor was higher than 2.5 standard deviations.There were 33 patients in the intermittent treatment group,including 2 children who had been treated with rhGH for 5 years,and1 patient who had been treated for 7 years.Thirty-three GHD patients of the same age group who were continuously receiving rhGH treatment during the first to third years were selected from the database of short stature people in this area as the continuous treatment group.All patients selected in both groups were meet the diagnostic criteria for growth hormone deficiency promulgated by the Pediatric Endocrine Society of the Chinese Medical Association in 2008.All patients with GHD were prepubertal before receiving rhGH treatment and patients with poor compliance were not included in this study.Collect age,sex,Ht,Ht SDS,BA,IGF-1,IGFBP-3 and other growth parameter indexes and records of adverse reactions in the two groups of patients,Disposal these data and use the statistical software to analyze the growth index changes of the intermittent treatment group and the continuous treatment group after rhGH treatment,and compare the effects between the two groups.Results:1.Comparison of general information between the two groupsThere were 21 males in the intermittent treatment group,accounting for63.6%,and 12 females,accounting for 36.4% according to statistical analysis.Their age ranged from 5.5 to 10.6 years,with an average age of 7.96± 1.57 years.There were 17 males in the continuous treatment group,accounting for51.5%,and 16 females,accounting for 48.5% according to statistical analysis.Their age ranged from 4.5 to 10.1 years,with an average age of 7.45 ± 1.61 years.There was no statistically significant difference in age,sex composition ratio,Ht,Ht SDS,Gv,BA,BA-CA,BA/CA,IGF-1,IGFBP-3 before the two groups of patients treated with rhGH(P>0.05).2.Comparison of growth indexes between two groups of patients after treatmentAfter 3 years of rhGH treatment in the continuous treatment group and the intermittent treatment group,there was statistically significant difference(P <0.05)in indexes like Ht,Ht SDS,Gv,BA,BA-CA,BA/CA,IGF-1,and IGFBP-3 which improved compared with those before treatment.3.Comparison of post-treatment effects between two groups of patientsThere were no group effects between the continuous treatment group and the intermittent treatment group on the indexes include Ht,Ht SDS,Gv,BA,BA-CA,BA/CA,IGF-1,and IGFBP3,and the difference has no statistically significant(P> 0.05).There were no significant adverse reactions occurred in the two groups of patientsbefore and after treatment.Conclusion:1.rhGH has a significant growth-promoting effect on GHD patients,and it is relatively safe.2.After continuous treatment with rhGH,children with intermittent treatment and children with long-term continuous treatment has the same curative effect on the growth parameter indexes include Ht,Ht SDS,and Gv.3.After rhGH continuous treatment,children with intermittent treatment and children with long-term continuous treatment have the same curative effect on the indexes include BA,BA-CA,BA/CA,IGF-1,and IGFBP-3.
