[Objective]In recent years,IgA nephropathy has been explored more and more,but its occurrence and development mechanism are still being explored.This study is to investigate podocytes,apoptosis inhibitors of macrophage(AIM),TGF-?1 is pathologically correlated with IgA nephropathy,summarizing the possible role of the three in IgA nephropathy.[Method]This study collected patients diagnosed with IgA nephropathy by renal puncture in Subei People's Hospital(excluded patients with other systemic severe primary diseases(such as heart,brain,liver,blood,lung),malignant tumors and chronic wasting diseases;combined with allergies Patients with secondary IgA nephropathy such as purpuric nephritis,lupus nephritis,hepatitis B-associated nephritis,diabetic nephropathy,hypertensive nephropathy,plasma cell myeloma,etc.can cause kidney-damaging diseases;hormones were used before renal biopsy to confirm the diagnosis,Patients with cytotoxic drugs and immunosuppressive agents;patients who have undergone renal replacement therapy or after kidney transplantation;patients with confirmed IgA nephropathy but incomplete clinical data).A total of 15 cases were finally included in the study group,including 4 males and 11 females.The pathological types were classified according to IgA nephropathy Oxford classification.The control group selected 6 normal kidney tissues from patients with kidney tumors in the same period.Immunofluorescence technology was used to detect the expression of podocyte markers nephrin,AIM and TGF-?1 in kidney tissue specimens,and observe the expression characteristics of the three.Analysis of the pathological correlation between the three and IgA nephropathy.SPSS 22.0 statistical software was used for data collation and statistical analysis,and the difference was statistically significant with p<0.05.[Result]A total of 15 cases were included in the study group,including 4 males and 11 females,and a total of 6 cases were included in the control group,3 males and 3 females.Based on the analysis of clinical data and research results,the following data were obtained:1.The podocyte marker nephrin was evenly distributed along the capillary loop in the renal tissue of the normal control group,and there was no obvious expression in the renal tubule interstitial.AIM and TGF-?1 were rarely expressed,while the patients in the study group had IgA nephropathy The expression of nephrin in kidney tissue was significantly reduced,while the expression of AIM and TGF-?1 in kidney tissue of patients with IgA nephropathy was significantly increased.2.In a study that further subdivided the research group into E0/E1 subgroups,it was found that podocyte density in patients with IgA nephropathy was negatively correlated with E0/E1(r=-0.599,p<0.05),that is,podocyte density Reduced,corresponding to the presence of endothelial cell hyperplasia in the kidneys of patients with IgA nephropathy.The expression of AIM in patients with IgA nephropathy is positively correlated with E0/E1(r=0.537,p<0.05),that is,the increase in expression of AIM corresponds to the proliferation of endothelial cells in the kidneys of patients with IgA nephropathy.There was no significant correlation between TGF-?1 and endothelial hyperplasia.Further regression analysis showed that the expression of AIM(p = 0.067)and TGF-?1(p =0.098)were not independent risk factors for endothelial cell proliferation in patients with IgA nephropathy,while podocyte density was an independent risk factor for endothelial cell proliferation in patients with IgA nephropathy(P <0.05).3.In a study that further subdivided the research group into T1/T2 subgroups,it was found that podocyte density in patients with IgA nephropathy was negatively correlated with T1/T2(r=-0.848,p<0.01),that is,podocyte density Reduced,corresponding to the severity of renal tubular atrophy or interstitial fibrosis in patients with IgA nephropathy.The expression of AIM(r=0.800,p<0.01)and TGF-?1(r=0.868,p<0.01)in patients with IgA nephropathy was positively correlated with T1/T2,that is,the increased expression of AIM and TGF-?1 corresponded to the kidneys of patients with IgA nephropathy The more severe the tubular atrophy or interstitial fibrosis.Further regression analysis found that podocyte density,AIM,and TGF-?1 expression were not independent risk factors for renal tubular atrophy or interstitial fibrosis.4.Further analysis of the correlation between podocyte density,AIM and TGF-?found that there is a significant correlation between the three,in which podocyte density is negatively correlated with AIM and TGF-?1,respectively,while AIM and TGF-?1was positively correlated.[Conclusion]Podocyte injury,AIM,TGF-?1 and IgA nephropathy have a certain correlation,and the three may also play a role through mutual influence,which may be used to explore the progress of IgA nephropathy and the future treatment direction.Provide more basis.
