Congenital auricular malformation is a common disease in otolaryngology.Studies show that auricular orthosis has achieved good correction effect in treatment of auricular malformation in early neonatal period,but there is no objective evaluation index for the specific effect at present,and there are few studies on the correction effect of over-age patient children.Therefore,this study intends to discuss the curative effect of auricular orthosis through auricular morphology measurement,and analyze the effect of different ages and different types of auricular malformation on the curative effect through various evaluation indexes.Objective: To explore the curative treatment effect of ear model orthosis for congenital auricular malformation in infants by comparing the auricular morphological data before and after treatment with ear model orthosis,and to discuss the effects of different ages and different malformation types on the curative effect by analyzing the comparison of treatment cycles,doctor's rating on the curative effect for the auricular appearance after treatment,parents' satisfaction with the auricular morphology after correction of auricular malformation(visual analogue scale(VAS)),incidence of adverse reactions and gender factors.Method: 86 healthy children suffering from unilateral congenital auricular malformation(one of helix malformation,hidden ear and cup-shaped ear)from October 2017 to October 2019 in outpatients of Otolaryngology Department of Hunan Children's Hospital were selected and included in thegroup.After informed consent forms were signed by their parents,they were corrected with EarWell auricular orthosis.After treatment,changes in auricular morphological data were compared,and the effects of different initial treatment ages and different types of auricular malformation on the curative effect were explored according to various evaluation indexes.Result:1?1.Changes of facial ear length,facial ear width,morphological ear length,morphological ear width and vertical distance between cranium and ears in the auricular morphological data before and after treatment with ear model orthosis were statistically significant(P < 0.05),and the changes of cranioauricular angle values before and after treatment were not statistically significant(P > 0.05).2? The average age of the included children was 18.69±12.39 weeks.They were divided into the 2-6 weeks group,the 6 weeks-3 months group and the 3-6 months group according to the initial treatment age,accounting for 29.07%,38.37% and 32.56% of the total samples respectively.The differences among different age groups in the treatment cycle,doctors' rating on the curative effect,parents' VAS score on curative effect and the presence and absence of adverse reactions were statistically significant(P < 0.05).3?The three types of auricular malformation included in the study were the helix malformation,the hidden ear and the cup-shaped ear,accounting for44.00%,44.00% and 12.00% respectively in the 2-6 weeks group,i.e.,the minimum age group.There was statistically significant difference between the groups in doctors' rating on the curative effect(P < 0.05).4 ? The age and type of auricle deformity were correlated with the treatment cycle,VAS score,efficacy rating and adverse reactions(P<0.05),and the sex was not correlated with the efficacy index(P>0.05).The difference of ear length before and after treatment was correlated with treatment period,VAS score and adverse reaction(P<0.05).Conclusion:1 ? Auricular orthosis can correct congenital auricularmalformation in infants effectively.2?Different initial treatment ages have an effect on the curative effect.The best effect is achieved when wearing of ear model orthosis is started within 6 weeks after birth,but auricular malformation in over-aged children aged within 6 months can also be corrected by prolonging the treatment period.3 ? In the 2-6 weeks group,among the three types of auricular malformation included in this study,the hidden ear is the one with the best rating for the auricular appearance after treatment,followed by the helix malformation,and then the cup-shaped ear.
