| Research purposes: To compare the safety and efficacy of sleeve lobectomy(SL)and pneumonectomy(PN)for central lung cancer by meta-analysis.The main indicators includes: postoperative complication rate,postoperative mortality,the tumor recurrence rate,metastasis rate,and 5-year survival rate.Research methods:An online search of the following databases : China National Kowledge Internet(CNKI),VIP database,Wanfang Database,Google Scholar,Embase,Pubmed,MEDLINE,Cochrane Library.All the non-randomized controlled study comparing sleeve lobectomy and total pneumonectomy published from January 2005 to December 2019 were collected.According to the inclusion criteria,literature was selected and scored.The required data were extracted : the postoperative complication rate,the postoperative mortality,the tumor local recurrence rate,and the 5-year survival rate.Meta analysis was performed by using Rev Man5.3 software.Result:A total of 18 literatures were enrolled in the meta-analysis,including 11 Chinese literatures and 7 English literatures.The total number of cases was 2909,the number of cases in the SL group was 1175 and the number of cases in the PN group was 1734.The postoperative complication rate was lower in the SL group than in the PN group,(OR=0.44,95% CI:0.28~0.71,P=0.0008)The former had lower postoperative mortality than the latter(OR=0.39,95% CI:0.19~0.67,P=0.001).But local tumor recurrence rate was not statistically significant(OR=1.30,95%CI:0.89~1.90,P=0.16).The 5-year survival rate of the former was higher than the latter(OR=2.02,95% CI:1.65~ 2.47,P<0.00001).Conclusion:SL compared with PN is safe and effective in the treatment of central lung cancer,and it can bring a longer survival. |
