Objective: The study aims to study Hepatocellular Carcinoma(HCC)by Gd-EOB-DTPA-enhanced MRI imaging technology and explore the differences in diagnostic effectiveness of MDCT,conventional extracellular contrast media(ECCM)enhanced MRI and Gd-EOB-DTPA-enhanced MRI in HCC patients and whether there is a difference in staging of HCC between different imaging methods.Methods: The image data of MDCT,conventional ECCM-enhanced MRI and Gd-EOB-DTPA-enhanced MRI in patients with hepatocellular carcinoma diagnosed in The Second Hospital University of South China from January 2018 to December 2019 were analyzed,the histopathological report after resection or biopsy of a lesion served as the gold standard,and a surrogate of follow-up or complementary imaging techniques in combination with clinical and paraclinical parameters was used in unresected lesions.The patients were divided into stages according to the criteria for diagnosis and treatment of primary hepatocellular carcinoma(2019 edition)respectively.Chi-square test or Fisher’s accurate test was used to statistical analysis,p<0.05 showed that there was statistical significance.Result: A total of 42 liver cancer patients were included in this study.There were total 142 hepatic nodules,and 90 nodules were HCC.1)Compared with MDCT and ECCM-enhanced MRI,Gd-EOB-DTPA-enhanced MRI showed significantly greater sensitivity[97.8%(139/142)] for hepatic nodules,higher than that of MDCT [78.8%(112/142)] and ECCM-enhanced MRI [84.5%(120/142)],(p<0.05).In particular,the sensitivity of nodules ≤3cm is higher than that of MDCT or ECCM-enhanced MRI,(p<0.05).However,there was no significant difference in the sensitivity of the three imaging methods in the diagnosis of > 3cm nodules,(p>0.05).2)The consistent rate between the diagnosis of hepatocellular carcinoma by Gd-EOB-DTPA-enhanced MRI and pathological results was 96.7%(87/90),higher than that by MDCT[56.7%(51/90)] and ECCM-enhanced MRI[70.0%(63/90)],with statistically significant differences(p<0.001),and the three imaging methods showed significant differences in the detect different numbers of hepatocellular carcinoma lesions,(p<0.05).3)The patients were divided into stages according to the criteria for diagnosis and treatment of primary liver cancer(2019 edition).Gd-EOB-DTPA-enhanced MRI showed significantly more accurate staging [80.9%(34/42)] in HCC.Moreover,In HCC patients with incorrect staging of MDCT and conventional ECCM-enhanced MRI,the staging was corrected after Gd-EOB-DTPA-enhanced MRI was added.The data were analyzed by a paired chi-square test.In staging among the three imaging methods was significant(p<0.05).Conclusion: Compared with MDCT and extracellular contrast media enhanced MRI,Gd-EOB-DTPA-enhanced MRI imaging technology has a higher sensitivity to the detection of hepatic nodules,especially to thedetection of Hepatocellular Carcinoma nodules,which can also improve the accuracy of staging of HCC and has an important guiding significance to the options for clinical therapy.
