BackgroundThe morbidity and mortality of malignant tumors in China are increasing year by year.Due to the lack of specific clinical symptoms and early diagnostic methods,many in the discovery has lost the best treatment opportunity.As early as 1863,German pathologist Virchow was first put forward the malignant tumor originated in chronic inflammation may be the hypothesis,then the more studies have found that C-reactive protein(CRP),vascular endothelial growth factor(VEGF)and interleukin-6(IL-6)not only play a very important role in chronic inflammation,but also may be involved in the occurrence and development of malignant tumors.ObjectiveTo explore the clinical value of CRP,VEGF and IL-6 in serum in patients with malignant tumors,and to analyze the relationship between these inflammatory factors and the diagnosis,stage and prognosis of malignant tumors,provide basis for the timely diagnosis of malignant tumors,to formulate a reasonable diagnosis and treatment plan,improve the prognosis,improve the survival rate and quality of life.MethodsFrom July 2017 to July 2019,a total of 77 newly treated malignant tumor patients are divided into malignant tumor group and 50 cases healthy people as control group were selected in the third affiliated hospital of Xinxiang Medical University.All the patients were pathologically diagnosed as the malignant tumor group,according to the AJCC staging criteria for stage I period,stage II period,stage III period,stage IV period,all patients in clinical information collected on admission(including hospitalization number,name,gender,age,diagnosis,staging,pathological type,etc.),to ensure the truthfulness and completeness of the information,and venous blood was collected from all subjects on an empty stomach in the early morning,the serum levels of CRP,VEGF and IL-6 in the two groups.Using SPSS 19.0 statistical software for statistical analysis of all data,measurement data using(??ąs),count data using(n),meet the homogeneity of variance using t test,multiple independent samples were compared using one-way ANOVA;and compared using?~2 test,draw the receiver operating characteristic(ROC)curve,survival curve drawing with Kaplan-Meier method,and prognostic analysis using Cox regression model,P<0.05 was considered as significant.To the clinical value of CRP,VEGF and IL-6 in serum in patients with malignant tumors,and to analyze the relationship between these inflammatory factors and the diagnosis,staging and prognosis of malignant tumor.Results1.Compared with the control group,the serum levels of CRP,VEGF and IL-6increased in malignant tumor group,the difference was statistically significant(P<0.05).2.In malignant tumor group,the serum levels of CRP,VEGF and IL-6 in the I-?period gradually increased,the difference was statistically significant(P<0.05).3.ROC curve analysis showed that:1)the diagnostic cut-off values of the serum levels of CRP,VEGF and IL-6 were 6.68mg/L?182.78 pg/ml?9.67 pg/mL respectively,sensitivity were 59%?43%?52%respectively,specificity were 86%?94%?92%respectively,Youden index were 0.42?0.37?0.44 respectively,AUC were 0.745(95%CI:0.662-0.828)?0.668(95%CI:0.576-0.760)?0.670(95%CI:0.576-0.764)respectively;2)the sensitivity of the serum levels of CRP+VEGF?CRP+IL-6?VEGF+IL-6?CRP+VEGF+IL-6 were 75%?76%?62%?82%respectively,specificity were 98%?94%?98%?94%respectively,Youden index were 0.73?0.71?0.60?0.76 respectively,AUC were0.818(95%CI:0.745-0.890)?0.886(95%CI:0.826-0.947)?0.782(95%CI:0.701-0.863)?0.905(95%CI:0.851-0.959)respectively.4.The results of the univariate analysis showed that CRP,VEGF and IL-6 were the related factors for prognosis of patients with malignant tumors.5.The COX multivariate analysis showed that CRP,VEGF and stage were independent risk factors for prognosis of patients with malignant tumors.Conclusions1.The serum levels of CRP,VEGF and IL-6 in patients with malignant tumor were significantly higher than those in healthy people,and the later the stage is,the higher the serum CRP,VEGF and IL-6 levels are,suggesting that serum CRP,VEGF and IL-6 levels are related to the stage of malignant tumor.2.The combined detection of serum CRP,VEGF and IL-6 is of great clinical value.3.The serum CRP,VEGF and IL-6 are all relevant factors affecting the prognosis of patients with malignant tumors,serum CRP,VEGF and stage are independent risk factors affecting the prognosis of patients with malignant tumors.
