BackgroundHeart failure is a group of complex clinical syndrome,which is the final stage of the development of various heart diseases,showing the characteristics of high disability rate,high mortality and poor prognosis.The incidence of coronary heart disease is gradually increasing.The incidence of Ischemic cardiomyopathy(ICM)heart failure caused by long-term myocardial ischemia,hypoxia or necrosis is also increasing,which gradually become the main cause of heart failure.Patients with ICM heart failure caused by myocardial infarction have higher mortality and worse prognosis.Recent studies have shown that Galectin-3(Gal-3)is a ?-galactose binding protein,which plays an important role in myocardial fibrosis and ventricular remodeling.Gal-3 is expected to become a new biomarker for the diagnosis of heart-related diseases in clinic.The exact role of Gal-3 in ICM heart failure caused by myocardial infarction is rarely studied at home and abroad.ObjectiveTo investigate the changes of plasma galectin-3(Gal-3)expression in Heart Failure(HF)Patients Caused by Ischemic Cardiomyopathy(ICM)After Myocardial Infarction(MI)and to explore its clinical significance.MethodsOur research included 4 groups: 29 HF patients Caused by ICM After MI(MI-ICM-HF group,all of them were patients with old myocardial infarction and they were diagnosed as coronary heart disease by coronary angiography,which met the diagnostic criteria of ICM and heart failure.);27 HF patients Caused by ICM non-MI(ICM-HF group,they all ruled out myocardial infarction and they were diagnosed as coronary heart disease by coronary angiography,which met the diagnostic criteria of ICM and heart failure);21 HF patients with non-ICM(HF group,they performed coronary angiography to rule out coronary heart disease,which met the diagnostic criteria of heart failure);22 control patients(control group,they performed coronary angiography to rule out coronary heart disease,echocardiography was normal and heart failure diagnosis was excluded.),these patients admitted in our hospital from 2018-07 to 2019-02.Plasma levels of galectin-3 and NT-proBNP were examined by ELISA;left atrial diameter(LAD),left ventricular end diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF)were measured by Echocardiography;correlation study was conducted by Spearman analysis.ResultsCompared with the level of Gal-3 in the four groups,the level of Gal-3 in the MI-ICM-HF group was the highest,P<0.01.Pairwise comparison of Gal-3 levels among the four groups: Gal-3 levels in MI-ICM-HF group were higher than those in ICM-HF group,HF group and control group,all P<0.01.Gal-3 levels in ICM-HF group and HF group were higher than control group,all P<0.01.There was no significant difference in Gal-3 level between ICM-HF group and HF group,P>0.05.After ROC analysis,the ROC AUC of Gal-3 in the diagnosis of Heart Failure is 0.989,which has a good diagnostic efficiency.The ROC AUC of Gal-3 in the diagnosis of Heart Failure patients caused by Ischemic Cardiomyopathy after Myocardial Infarction is 0.753,which has a certain diagnostic efficacy.According to the comparison of cardiac function parametes among the four groups,there were significant differences in LVEDD and LVEF,P<0.01.Pairwise comparison of LVEDD levels among the four groups: LVEDD levels in MI-ICM-HF group were higher than those in ICM-HF group,HF group and control group,all P<0.05.LVEDD levels in ICM-HF group and HF group were higher than control group,all P<0.05.There was no significant difference in LVEDD level between ICM-HF group and HF group,P>0.05.Pairwise comparison of LVEF levels among the four groups: LVEF levels in MI-ICM-HF group were lower than those in ICM-HF group,HF group and control group,all P<0.05.LVEF levels in ICM-HF group and HF group were lower than control group,all P<0.05.There was no significant difference in LVEF level between ICM-HF group and HF group,P>0.05.Spearman correlation analysis indicated that in MI-ICM-HF group,galectin-3 was positively related to LVEDD,negatively related to LVEF,all P<0.05,and no obvious correlation to LAD,P>0.05;while in the other three groups,galectin-3 had no obvious correlation to those LAD,LVEDD,LVEF,P>0.05.ConclusionsThe expression of plasma galectin-3 in HF patients Caused by ICM After MI was significantly increased.Gal-3 has a good diagnostic efficacy for Heart Failure,and Gal-3 has a certain diagnostic efficacy for Heart Failure patients caused by Ischemic Cardiomyopathy after Myocardial Infarction.Gal-3 can reflect the cardiac structure changes of HF patients Caused by ICM After MI.
