Objective:Use grounded theory to extract the influencing factors of the integrated chronic disease management service effect,clarify the logical relationship between the influencing factors,calculate the weight of the influencing factors through the entropy weight method and the G1 method,and find the key factors in combination with the system dynamics method Influencing factors,suggestions are made from the perspective of key factors to provide an effective basis for further improving the comprehensive prevention and control of chronic diseases in China.Methods:This article adopts semi-structured interviews to obtain the original data,and then uses grounded theory to encode the interview text at three levels,and extracts the factors that influence the effectiveness of integrated chronic disease management services.Then obtain the relevant data in the form of questionnaire survey,use the entropy method to determine the objective weight of the factors affecting the effect of integrated chronic disease management services,and use the G1 method to determine the subjective weight of the factors affecting the effect of integrated chronic disease management services.Weights.Based on this,construct the causality diagram of the influencing factors and the stock flow diagram of the integrated chronic disease management service,establish the system dynamics model according to the relationship between the influencing factors,and input the weights determined by the G1 method and the entropy weight method and other relevant parameters Model to simulate the impact of various influencing factors on the effectiveness of integrated chronic disease management services.Finally,through single-strategy simulation and variable control methods,the main influencing factors are determined,and countermeasures and suggestions are put forward for the improvement of integrated services for chronic diseases.Results:The first-level index weights of the influencing factors of the integrated chronic disease management service effect determined by the entropy weight method and the G1 method are as follows,in which the comprehensive weight of institutional guarantee is 0.29,the comprehensive weight of health education and intervention is 0.169,and the comprehensive weight of chronic disease treatment and management The comprehensive weight is 0.17,the comprehensive weight of healthy environment support is 0.147,and the comprehensive weight of chronic disease patient factors is 0.224.The ranking of the secondary indicators of the influencing factors of the integrated chronic disease management service effect determined by the entropy weight method and the G1 method,the top five are the comprehensive weight of the self-management ability of chronic disease patients is 0.053,and the comprehensive weight of the early screening of high-risk groups is 0.051.The comprehensive weight of standardized management of chronic diseases in the community is 0.05,the comprehensive weight of chronic disease patient compliance is 0.048,and the comprehensive weight of chronic disease and risk factor monitoring capabilities is 0.047.Conclusion:The effect of integrated chronic disease management services is affected by many factors and needs to be completed through the joint cooperation of various subjects.The government should play a leading role in the integrated management of chronic diseases,drive the horizontal and vertical integration of chronic disease management through benefit sharing,improve the relevant funding guarantee mechanism,allocate public health and medical funds in a targeted manner,and ensure the service capabilities of chronic disease management institutions.Professional health institutions should play the role of lead agency,carry out chronic disease management cooperation and assistance,rely on the community,establish a chronic disease management professional team,and rely on information sharing for chronic disease monitoring and early intervention.Chronic patients should improve their self-management ability,develop a healthy lifestyle,maintain a positive and optimistic attitude,and improve self-efficacy.
