| Objective: To investigate the linear relationship between the hemodynamic indexes of systolic flow and diastolic flow and carotid bifurcation intima thickness(BIF-IMT)and find the inflection point value,to provide a basis for further exploring the mechanism of carotid artery disease and the application of hemodynamic indicators in carotid disease.Methods: A total of 3487 patients who underwent carotid ultrasound examination in the ultrasound department of Maanshan Hospital from December 2013 to December2018 were collected as study subjects.We conducted questionnaire surveys,laboratory tests and ultrasound imaging examinations on all subjects.The questionnaire survey includes: demographic characteristics such as age and gender,habits such as Smoking and drinking,as well as past history;laboratory tests include: blood lipid,blood glucose and renal function indicators;carotid artery ultrasound examination includes:carotid intimal thickness,presence or absence of plaque,location of plaque,size and thickness of plaque,and hemodynamic indicators(PSV,EDV,RI).The differences of blood biochemical indexes and hemodynamic indexes in carotid intimal lesion group were compared.Multi-factor analysis was performed by fitting the left and right BIF-IMT and hemodynamic smooth curves,and group according to the curve intercept points.The trend of BIF-IMT and hemodynamic indexes in different groups were tested by linear regression analysis model.Results: 1.Analysis of general demographic characteristics showed that the average age of subjects was 59.39 ± 11.44 years old,including 1749 males(50.16%)and 1738females(49.84%).There were 2297 cases(65.87%)in the RBIF-IMT(<1.0mm)group,888 cases(25.47%)in the RBIF-IMT [1.0mm,1.50mm)group,and 302 cases(8.66%)in the RBIF-IMT(≥1.5mm)group.There were 2215 cases(63.52%)in the LBIF-IMT(<1.0mm)group,987 cases(28.31%)in the LBIF-IMT [1.0mm,1.5mm)group,and285 cases(8.17%)in the LBIF-IMT(≥1.5mm)group.2.Relationship between left hemodynamics and LBIF-IMT: LBIF-IMT decreased with the increase of LCCA PSV(β=-0.039,P<0.001)and EDV(β=-0.046,P<0.001);LBIF-IMT increased with the increase of LCCA PSV(β=0.021,P=0.006).3.Relationship between right hemodynamics and RBIF-IMT: As the RCCA PSV(β=-0.060,P<0.001)and EDV(β=-0.058,P<0.001)increased,the RBIF-IMT decreased;as the RCCA PSV and EDV increased,the RBIF-IMT decreased first(β=-0.045,P<0.001)(β=-0.086,P=0.002)and then increased(β=0.075,P<0.001)(β=0.063,P<0.001).4.Relationship between hemodynamics of different genders and BIF-IMT: Male LCCA PSV was in the range of 0-150cm/s,and LBIF-IMT decreased(β=-0.061,P=0.016).Male LCCA EDV was in the range of 0-50cm/s,with the same trend(β=-0.051,P=0.034).Male LICA PSV was in the range of 0-150cm/s,whether adjusted or unadjusted,LBIF-IMT increased(β=0.012,P<0.001);female LBIF-IMT decreased with increasing of LCCA PSV(β=-0.039,P<0.001)and EDV(β=-0.050,P<0.001).In the range of 0-40cm/s,as the LICA EDV increased,LBIF-IMT decreased(β=-0.038,P<0.001).Male right common carotid artery PSV(β=-0.035,P<0.001)and EDV(β=-0.041,P<0.001)were in the range of 0-150cm/s and <50cm/s and right carotid artery PSV(β=-0.049,P<0.001)and EDV(β=-0.088,P<0.001)were in the range of <70cm/s and <25cm/s,the RBIF-IMT decreased.The female right common carotid artery PSV(β=-0.062,P=0.003)and EDV(β=-0.053,P=0.017)were in the range of 0-150cm/s and 0-50cm/s,respectively,RBIF-IMT decreased.When the right internal carotid artery PSV was in the range of 0-150cm/s,the RBIF-IMTincreased(β=0.065,P<0.001),whereas the right internal carotid artery EDV was in the range of <40cm/s,the RBIF-IMT decreased(β=-0.064,P=0.011).Conclusion: 1.In the study population,the systolic and diastolic flow velocities of the left and right common carotid arteries and the intima thickness of the left and right carotid sinus showed a negative relationship.The left internal carotid artery systolic flow velocity and diastolic flow velocity and the left carotid sinus intima thickness showed a positive relationship,and the right internal carotid artery systolic flow velocity showed a negative relationship at <70cm/s,and positive relationship at≥70cm/s.2.In the male and female,the left common carotid artery systolic and diastolic flow rate and left carotid sinus intima thickness showed a negative relationship;male left internal carotid artery systolic and diastolic flow rate and left carotid sinus intimal thickness showed a positive relationship;female left internal carotid artery showed a a negative relationship in the range of <40cm/s,and a positive relationship in the range of ≥40cm/s.3.when the systolic flow rate was <70cm/s,male the right internal carotid artery showed a negative relationship and showed a positive relationship in range of ≥70cm/s,and the right internal carotid artery showed a negative relationship when the diastolic flow rate was <25cm/s;female right internal carotid artery showed a negative relationship in range of <40cm/s of diastolic flow rate. |
