Objective: to analyze the clinical characteristics of Zhiyun Yihui management system in diabetic inpatients,To explore the role of Zhiyun Yihui management system in the management of diabetes mellitus,and to provide reference for promoting more scientific and reasonable blood glucose monitoring and scientific research;to compare the clinical efficacy and safety of Sanhuang Erdan Jiangtang Decoction on type 2 diabetes patients with deficiency of both qi and Yin and stasis,to explore the mechanism of its role,and to provide basis for the prevention and treatment of type 2 diabetes by traditional Chinese medicine.Method: 1.To collect the clinical data of diabetes patients in the Department of Endocrinology,the First Affiliated Hospital of Anhui University of traditional Chinese medicine from September 1,2019 to December 31,2019,and analyze the clinical characteristics of the application of Zhiyun Yihui management system in diabetes patients.2.Select 60 type 2 diabetes patients with deficiency of both qi and Yin and stasis in our department from December 1,2018 to November 30,2019,and 30 in the study group and the control group respectively.The control group was treated with routine basic therapy,and the study group was treated with Sanhuang Erdan Jiangtang Decoction for 6 weeks on the basis of the western medicine group.The body mass index,blood glucose,blood glucose fluctuation,blood lipid,TCM syndrome score,TCM syndrome total curative effect and adverse reactions were statistically analyzed before and after treatment in the two groups.Results: 1.Application analysis of Zhiyun Yihui management system in diabetes inpatients(1)The clinical characteristics of diabetic inpatients: from September 1,2019 to December 31,2019,there were 580 diabetic inpatients,225 males and 355 females;the peak age of the disease was 51-60 years old;the syndrome type of traditional Chinese medicine was mostly diagnosed as deficiency of both qi and Yin and stasis;the application of traditional Chinese medicine was most commonly used as tonic drugs.(2)Analysis of the clinical application of Zhiyun Yihui management system: the total number of blood glucose monitoring of Zhiyun Yihui management system is 22890.With the use of Zhiyun Yihui management system,the number of blood glucose monitoring per month is on the rise,but the per capita blood glucose monitoring is not high,and the average per capita blood glucose monitoring is 3.3 times per day.The most frequent blood glucose monitoring is fasting blood glucose and 2 hours after breakfast,4843 times and 4673 times,respectively There was less blood glucose monitoring in the morning and before lunch;the fasting blood glucose before treatment was 9.76±3.18mmol/l,the blood glucose 2 hours after breakfast was 14.63±4.02mmol/l,the blood glucose before lunch was 12.27±3.79mmol/l,the blood glucose 2 hours after lunch was 15.07±4.61mmol/l,the blood glucose before dinner was 12.25 ± 3.61mmol/l,the blood glucose 2 hours after dinner was 14.84 ±4.42mmol/l,the blood glucose before sleep was 11.89 ± 3.17mmol/l;the blood glucose in the morning was 8.3 1±2.89mmol/l;during the treatment,the incidence of hypoglycemia was less,42 times in total,the most time was before lunch;after treatment,the fasting blood glucose value was 6.41±1.42mmol/l,the blood glucose value was 10.19±2.33mmol/l 2 hours after breakfast,8.35±2.63mmol/l before lunch,11.35±2.85mmol/l 2 hours after lunch,8.76±2.49mmol/l before dinner,and 10.53 hours after dinner After treatment,the blood glucose was significantly lower than that before treatment.2.Comparison between the study group and the control group:(1)Comparison of the total effect of symptoms: after treatment,the study group and the control group have curative effect,and the study group and the control group have more advantages,P<0.05.(2)TCM symptom score comparison: after treatment,the symptom score of the study group and the control group was lower than before treatment(P<0.05);the symptom score of the study group was lower than that of the control group,the difference was statistically significant(P<0.05).(3)Comparison of FPG,2h PG and Hb A1c: after treatment,FPG,2h PG and Hb A1 c in the study group and the control group decreased compared with those before treatment(P<0.05);after treatment,FPG,2h PG and Hb A1 c in the study group were lower than those in the control group(P<0.05),with statistical significance.(4)Comparison of SDBG,PPGE and LAGE: after treatment,SDBG,PPGE and LAGE in the study group decreased compared with that before treatment(P<0.05);SDBG and LAGE in the control group decreased compared with that before treatment(P<0.05);PPGE in the control group showed no significant difference compared with that before treatment(P>0.05);after treatment,the results showed that SDBG,PPGE and LAGE in the study group decreased more significantly(P<0.05).(5)Comparison of TC,TG,LDL-C and HDL-C: after treatment,the levels of TC and LDL-C in the study group and the control group were lower than that before treatment,and the levels of HDL-C were higher,P<0.05;the TG in the study group was lower than that before treatment,P<0.05,and the TG in the control group was not statistically significant,P>0.05;after treatment,the levels of TC,TG and LDL-C in the study group were lower than that in the control group,and the levels of HDL-C were higher than that in the control group,P<0.05.(6)BMI comparison: there was no significant difference between the two groups after treatment and before treatment(P>0.05).(7)Safety comparison: there was no obvious abnormal blood routine and liver and kidney function between the study group and the control group after treatment,P>0.05;there were two hypoglycemia patients in the study group and three hypoglycemia patients in the control group during the treatment,P>0.05,there was no statistical significance between the two groups.Conclusions: 1.Zhiyun Yihui management system is an important and effective tool for diabetes education;it can increase the authenticity and reliability of blood glucose monitoring results,improve clinical work efficiency,and improve the blood glucose monitoring rate of patients;it can optimize the blood glucose monitoring structure,assist the blood glucose standard,reduce the incidence of hypoglycemia,and then delay the diabetes related complications.In clinical,the blood glucose monitoring before Chinese meal and dinner is less,so we should Increase the pre meal blood glucose monitoring.2.Sanhuang Erdan Jiangtang decoction can better improve the clinical symptoms of patients,improve the level of blood glucose and blood lipid in patients with type 2 diabetes,and reduce the fluctuation of blood glucose.3.Sanhuang Erdan Jiangtang recipe does not cause liver and kidney damage,does not increase the incidence of hypoglycemia,has high clinical safety,and is worth popularizing.
