Objective: To investigate the effectiveness of DKI(Diffusion Kurtosis Imaging)in the diagnosis of idiopathic Parkinson's disease and analyze the correlation with clinical progress.Methods:Thirty-five patients clinically diagnosed with Parkinson's disease in Xinjiang Second Clinical Hospital were selected as the case group,and 20 healthy control patients treated in our hospital during the same period were collected as the control group.All subjects underwent MRI routine scanning and DKI sequence scanning on a 3.0T magnetic resonance imaging machine.The mean kurtosis of bilateral substantia nigra,red nucleus,globus pallidus,thalamus and anterior and posterior cingulate gyrus were measured on the postprocessed DKI sequence respectively.(MK)value,anisotropic fraction(FA)value,Axial kurtosis(AK)value,radial kurtosis(RK).The average values of MK,FA,AK and RK in the brain of the case group and the control group were compared between groups by using two independent sample t tests to meet the normal distribution.The average values of MK,FA,AK and RK between the two groups were compared to find imaging indexes with differences in brain regions.Patients in the case group were divided into early Parkinson's disease group(13 patients)and advanced Parkinson's disease group(22 patients)to find out whether MK and FA values in DKI were correlated with clinical course progression.Receiver operating characteristic curve(ROC curve)was used to evaluate the diagnostic efficiency of various parameters in substantia nigra for PD.Results: The MK values of bilateral substantia nigra and red nucleus in the case group were significantly higher than those in the control group,with statistical difference(P<0.05),while FA values were lower than those in the control group,with statistical difference(P<0.05).AK and RK values had no significant statistical difference(P>0.05).MK values of bilateral globus pallidus and thalamus in the case group were higher than those in the control group,with statistical difference(P<0.05),FA values were slightly lower than those in the control group,with statistical difference(P<0.05),AK and RK values were not significantly different from those in the control group(P>0.05).The MK values of bilateral anterior cingulate gyrus and right posterior cingulate gyrus in the case group were lower than those of the control group,with statistical difference(P<0.05).FA,AK and RK values were not significantly different from those of the control group(P>0.05).MK,FA,AK and RK values of the left posterior cingulate gyrus were not significantly different from those of the control group(P > 0.05).No significant correlation was found between the parameters of bilateral substantia nigra,red nucleus,globus pallidus,thalamus MK and FA in early PD group and advanced PD group(P>0.05).MK value has diagnostic value for substantia nigra changes compared with other parameter values,and AUC is 0.70.Conclusion: MK and FA values in DKI parameters can be used as early biomarkers in the process of nerve degeneration.MK and FA values in DKI are not suitable for evaluating the progress of PD patients.
