| Objective: investigate the changes and correlation of serum cystatin C and BNP levels in patients with chronic heart failure with different ejection fraction,and evaluate the clinical significance of them in the severity of heart failure and prognosis.Methods:339 patients with chronic heart failure admitted to the heart center of the First Affiliated Hospital of Xinjiang Medical University from January to December 2017 were selected as the heart failure group.According to left ventricular ejection fraction(LVEF),the heart failure group was divided into three subgroups: diastolic dysfunction group(HFpEF group),intermediate ejection fraction group(HEmrEF group)and ejection fraction reduction group(HErEF group).126 healthy persons(control group).Serum levels of CysC and BNP were statistically analyzed.Results: 339 patients,165 males and 174 females,with an average age of(57.96±8.08),were included in the heart failure group.116 patients were in diastolic dysfunction group,114 in intermediate ejection fraction group and 109 in decreased ejection fraction group.There were 126 cases in the control group,55 males and 71 females,with an average age of(56.80±4.09)years.There was no significant difference in age,blood lipid,liver function and blood glucose between the two groups(P > 0.05).Compared with the control group,the LVEF and E/A of HFmrEF group and HFrEF group were significantly lower than those of the control group(P <0.05).LVEDd of HFmrEF and HFrEF group were significantly higher than that in the control group(P < 0.05).The levels of CysC and BNP in each subgroup of heart failure were higher than those in the control group,the difference was statistically significant(P< 0.05).In HFpEF group,the area under CysC ROC curve was 0.01,P=0.000,which was statistically significant.In HFmrEF group,the area under CysC ROC curve was 0.621,P=0.262,no statistical significance.In HFrEF group,the area under CysC ROC curve was0.828,P=0.002,which was statistically significant.Pearson correlation analysis showed that there was a positive correlation between CysC and BNP in each subgroup of CHF patients,(P < 0.05).Conclusion: Serum BNP and CysC are good indexes to evaluate the degree of CHF and ventricular remodeling.The combined monitoring and analysis of the changes in the condition of patients with heart failure not only reflects the changes of heart function,but also reflects the changes of heart structure. |
