Objective:To investigate the effect of Targetl-directed fluid therapy on postoperative rehabilitation of morbid obesity patients.Methods:Sixty severely morbid obesity patients undergoing laparoscopic sleeve gastrectomy were selected,including 28 males and 32females,aged 30 to 55 years,BMI 40-45 kg/m2,ASA?or III,using random number table Methods were divided into two groups of 30 patients each:target-directed fluid therapy group?group G?and conventional fluid therapy group?group C?.During the operation,HR,MAP,SpO2,BIS and PETCO2 were continuously monitored.Group G also used the FloTrac/Vigileo monitoring system to monitor stroke volume?SV?and stroke volume variation?SVV?.In group C,the fluctuation range of HR and MAP was not more than 20%before surgery,and urine output was>0.5 ml·kg-1·h-1.In group G,the stroke variability?SVV?was?13%.The crystal liquid is compound sodium lactate,and the colloid liquid is 6% hydroxyethyl starch 130/0.4.The intraoperative blood loss,the amount of infused crystal fluid,the amount of colloidal fluid,the total infusion volume,urine volume,and the use of vasoactive drugs were recorded.The post-extubation time,the first post-exhaust time,and post-hospitalization time were recorded.Cardiovascular complications,pulmonary complications,and renal impairment were recorded.The GIQLI scale was used to assess the quality of life of the two groups before and after surgery at 2,5,10,and 16 weeks.Results:Compared with group C,the amount of intraocular lens fluid,total infusion volume and urine volume in group G were significantly reduced?P<0.05?,and the time of first exhaust and postoperative hospital stay were significantly reduced?P<0.05?.The incidence of pulmonary complications was significantly reduced?P<0.05?.The GIQLI score of group G was significantly higher than that of group C at 2,5,and 10 weeks after operation?P<0.05?.There was no significant difference in the amount of colloidal fluid,the rate of vasoactive drugs,the time of tracheal tube removal,and the damage of renal function between the two groups of patients.Conclusion:The goal-directed fluid therapy based on the FloTracVigileo monitoring system can promote postoperative rehabilitation of morbid obesity patients and has certain clinical value.
