Objective: To compare the effects of different surgical methods on the prognosis of pancreatic body and tail cancer,and to provide a better choice for surgical procedures.Methods:Retrospectively analyzed the clinical data of 59 patients undergoing radical surgery for pancreatic body and tail cancer in the Pancreatic Surgery Department of the First Affiliated Hospital of Xinjiang Medical University from January 2014 to June 2017.According to different surgical methods,patients were divided into RAMPS group(radical antegrade modular pancre-atosplenectomy,RAMPS)and traditional surgery group.Patients in the RAMPS group were treated with radical antegrade modular pancreatosplenotomy,and patients in the traditional surgery group were treated with traditional pancreatic body and tail + splenectomy.The intraoperative and postoperative indexes of the two groups were compared,and the prognosis was compared.Results: The operations of 59 patients were successfully completed,and there were no deaths during hospitalization.The average operation time: RAMPS group: 456 min,traditional operation group 501 min,P> 0.05;intraoperative average blood loss: RAMPS group: 403 ml,traditional operation group 592 ml,P <0.05.Average postoperative hospital stay: 22.4days in the RAMPS group and 22.1 days in the traditional surgery group,P> 0.05.59 patients were interviewed after operation,and the follow-up period was 1-24 months.There was no difference in the recurrence rate between the two groups of subjects,and the difference in recurrence patterns: RAMPS surgery can reduce the postoperative local recurrence rate and liver metastasis rate;all patients who died during follow-up were due to tumor progression.The median survival time was 15 months in the RAMPS group and12 months in the traditional surgery group,with Log Rank P = 0.005.Conclusion:Compared with traditional pancreatectomy,RAMPS surgical approach is safe and feasible in the treatment of pancreatic body and tail cancer,which can reduce the risk of bleeding in patients during operation,reduce local recurrence and liver metastasis after operation,and prolong the operation of patients Survival period,improve patient prognosis.
