Pathological Data And Survival Status Of Patients With Primary Breast Lymphoma Were Analyzed

Objective: To explore the clinical features,treatment and possible prognostic f actors of primary breast diffuse large B-cell lymphoma(PB-DLBCL).Methods:The clinical data of 32 newly treated PB-DLBCL patients who were treated in the affi liated Cancer Hospital of Xinjiang Medical University from January 2009 to June2019 were analyzed retrospectively,and their clinical features,pathological features,treatment regimen,curative effect and possible prognostic factors were analyzed.Re sults:Among the 32 patients,there were 31 female and 1 male,with a median age of 45 years old.Painless breast mass was the first symptom in 28 cases,local pa in in 4 cases and B symptom in 4 cases.The clinical stages included 15 cases of IE stage(46.9%),7 cases of IIE stage(21.9%)and 10 cases of IVE stage(31.2%).32 patients were further classified,7 cases were from germinal center and 25 cases were from non-germinal center non-GCB.The treatment included chemother apy only(n=8),operation plus chemotherapy(n=15),sequential radiotherapy(n=9),imm unochemotherapy(n=20)and central nervous system prophylaxis(n=18).The media n follow-up period was 19.5 months,11 cases died(34.4%),the total 5-year survi val OS rate was 43.5%,and the 5-year progression-free survival PFS rate was 40.1%.Univariate analysis showed that IPI score,AnnArbor stage,immunochemothera py,number of completed cycles of first-line chemotherapy,efficacy of chemotherap y and central nervous system prophylaxis were correlated with OS(P < 0.05),IPI score,AnnArbor stage,immunochemotherapy,number of completed cycles of firstline chemotherapy,efficacy of chemotherapy and central nervous system prophylaxi s were correlated with PFS(P < 0.05).Multivariate analysis showed that IPI score(P = 0.039),number of completed cycles of chemotherapy(P = 0.032),immunoc hemotherapy(P = 0.027)and central nervous system prophylaxis(P = 0.033)were closely related to OS.IPI score(P = 0.002),immunochemotherapy(P = 0.036),c hemotherapy efficacy(P = 0.025)and central nervous system prophylaxis(P = 0.035)were closely related to PFS.Conclusion:The prognosis of primary breast lymphoma is good.At present,the main treatment mode is chemotherapy combined with immunotherapy and local radiotherapy based on anthracycline antineoplastic drugs.Immunochemotherapy and central nervous system prophylaxis can benefit the survi val of patients.
