Objective:To explore the clinicopathological characteristics and the risk factors affecting metastasis and prognosis of gastric neuroendocrine neoplasm.Method: The clinical and pathological features,diagnosis and treatment,and prognosis of 120 patients with gastric neuroendocrine tumors were retrospectively analyzed.SPSS24.0 software was used for statistical analysis of the data,Kaplan.Meier survival analysis was used to study the prognostic indicators,and Cox regression analysis was performed on meaningful indicators to find independent risk factors affecting prognosis.P<0.05 considered the difference to be statistically significant.Results:(1)The average age of 120 patients was57.2±8.7 years old,and the male-to-female ratio was 2.08:1,most of them were single lesions 63.3%(76/120),and 48.7%(37/76)were more common in the gastric cardia area,The general type is mainly ulcer type,about 50.0%(60/120).(2)21 were NET(17.5%),10 were NET G3(8.3%),59 were NEC(49.2%),and 30 were MANEC(25%).The size of the lesion was 0.1 cm-12 cm.The patient had almost no special clinical symptoms,and the most common clinical manifestation was abdominal pain(53.3%).(3)Patient age,tumor diameter,number of lesions,pathological grade,and classification are significantly related to gastric NEN metastasis.Multivariate analysis of pathological classification and tumor diameter are independent predictors of metastasis.(4)The median survival time of all patients was 33 months,age>60 years(P= 0.001),tumor diameter>5cm(P=0.000),with distant metastasis(P=0.000),MANEC(P=0.000),NEC(P=0.02)is an independent risk factor for poor prognosis of gastric NEN.(5)The prognosis of patients with NEC and MANEC is poor,but the independent risk factors that affect the prognosis of the two are different,with distant metastasis,tumor diameter>5cm,and age>60 years old are independent indicators of poor prognosis for NEC.However,for MANEC,age>60 years and lesion diameter>5cm are independent risk factors.Conclusion:(1)Gastricneuroendocrine tumors are more common in men with pancreas,mainly gastric cardia area,single,ulcer type lesions;(2)Tumor diameter and pathological classification are significantly related to the metastasis of gastric neuroendocrine tumors and are independent predictors;(3)Patients with ages greater than 60 years old,large tumor diameters,distant metastases,and pathology classified as MANEC and NEC have poor prognosis.
