Objective:To summarize and analyze the clinical history data of 193 patients with chronic subdural hematoma treated by endoscopic assisted and burr hole drainage,and retro spectively analyze the clinical effects of two surgical treatments.Methods:From February 2007 to February 2017,eligible 193 patients of chronic subdural hematoma were choosen to be the research object from Affiliated Hospital of Taishan Medical College.We divide the 193 patients into endoscopic group and drilling group according to the different surgical methods,use SPSS19.0 to analyse the differences between the two groups of patients in age,gender,brain trauma,the first symptom past history,subdural hematoma volume,preoperative GCS scores,revised version of the Markwalder neural function grading,midline shift,CT brain hematoma density,postoperative drainage time,postoperative GCS score and postoperative revised version of the Markwalder neural function grading,postoperative complications and the recurrence rate,comparative analyse the advantages and disadvantages of the two types of operation and their relative indications.Results:1.Two groups of patients had no significant difference in general information(such as age,gender,craniocerebral trauma,first symptom,past medical history and subdural hematoma volume)(P>0.05).2.Two groups of patients had better postoperative neurological function improvement than before operation(P<0.05),but the endoscopic group had no obvious advantages in improving neurological function than the drilling group(P>0.05).3.The patients in the endoscopic group were shorter than the drilling group,and the postoperative recurrence rate was low,but the operation time was prolonged(P <0.05).Conclusion:Compared with burr hole drainage,endoscopic assisted treatment of chronic subdural hematoma can shorten the hospitalization time,reduce the recurrence rate,reduce the incidence of complications such as cerebral contusion and traumatic subarachnoid hemorrhage,but prolong the operation time.There was no significant difference between the two methods in improving the neurological function of the patients.In contrast,endoscopic assisted therapy is more suitable for patients with chronic subdural hematoma,such as hematoma and septum and CT with high density and mixed shadows.burr hole drainage is more suitable for elderly patients.
