BackgroundNursing work i5 an important part of medical service work.The quality of nursing work,patient satisfaction,work efficiency of nursing staff,and nurse turnover rate largely depend on the level of work experience of nursing staff The work experience is the individual’s psychological feelings about the whole work process and related aspects,generally including self-development prospects,job satisfaction,work stress feelings and turmover intention.At present,the shortage of nurses and the high turnover rate are an interrational problem.Therefore,as an important factor affecting the stability of the nursing team and the quality of nursing services,the nurses’ work experience should be highly valued by health administrative decision makers and hospital administrators.And it also has high practical value.The American Nursing Society ranks the job satisfaction of clinical nurses as one of the indicators of sensitivity to care quality.Judging from the existing literature,there are few studies cm the sensitivity of nurses’ work in China,so they have better novelty.ObjectivesThis study was conducted a questionnaire survey on nursing staff at various levels of medical institutions in Shandong Province to understand the level of their work experience,analyze the influencing factors,and propose relevant countermeasures,to provide reference for improving nursing stafif working experience level,improving nursing quality and stabilizing nursing staff.Materials and MethodsThe research data is derived from the data of the“Nursing Staff Survey Form" in Shandong Provincial Health Service Survey Database.According to the principle of cost-effective,samples were selected by stratified random sampling method,and the research subjects were on-the-job nursing staff in medical and health institutions in Shandong Province.The survey included two parts:the general condition of the nurse and the work experience scale.The research content is the basic situation of nursing staff in medical institutions in Shandong Province,the status quo of nursing staff’s work experience and the influencing factors.SPSS24.0 software was used for data analysis.Main ResultsA total of 757 nursing staff were surveyed,of which 98.0%were women,74.3%were under 40 years old,85.2%were college graduates or above,79.1%were married,and 87.9%were under intermediate titles.Registered nurses accounted for 97.2%,those with a working life of 10 years and above accounted for 60.7%,and those with formal preparations accounted for 73.3%.The number of nursing staff from primary and non-primary medical institutions accounted for 42.1%and 57.9%respectively.72.3%of nursing staff work 40 hours or more per week.From the perspective of job experience,the average scores of the five dimensions of nursing staffsself-development evaluation,external satisfaction,internal satisfaction,work stress evaluation and turnover intention were 17.9±2.2,17.7±4.3,1 7.7±4.1,1 5.3±5.0,9.3±5.2respectively,which shows that self-development evaluation,extenlal satisfaction and internal and external satisfaction are higher,work pressure is higher,and turnover intention is lower.From the perspective of self-development evaluation of nursing staff,79.0%and 20.3%of the people evaluated“good,and“general,respectively;Univariate analysis showed thatthe influence factors of each dimension in work situation of different research objects are different.Among all factors,the level of education(P=0.023),whether to be permanent staff(P=0.005),management position(P=0.002),monthly night shifts(p=0.030),annual income(p=0.001)can affect the development of self-evaluation.From the external satisfaction dimension of nursing staff,70.0%and 28.8%respectively rated "satisfaction" and "general";The management position(P<0.001),monthly night shift(P=0.035),annual income(P=0.002)can affect the extrinsic satisfaction,From the perspective of the internal satisfaction of nursing staff,57.7%and 37.4%of the people rated"satisfaction" and "general" respectively;The influence factors of the inner satisfaction are whether to be permanent staff(P=0.004),management position(p<0.001),monthly night shift(P=0.004)and the annual income(P<0.001).From the perspective of the stress perception of nursing staff,40.3%and 49.3%of the pressure perceptions were "high,and "general,respectively;Among all factors,age(P<0.001),marital(P=0.018),level of education(p<0.001),working years(p=0.004),whether to be permanent staff(P=0.024),the department(P=0.016),whether working in primary health care institutions(P<0.001),the institutions of work(P<0.001)and the annual income(P=0.009)can influence the feeling of the work pressure.From the perspective of the turnover intention of nursing staff,52.8%and 36.9%of the turnover intentions are "low" and“general,respectively;The influence factors of the feeling of turnover intention are management position(P=0.006),monthly night shift(P=0.035)and the annual income(P=0.006).Multiple factor analysis revealedin terms of the development of self-evaluation,the self-development evaluation satisfaction of those with a bachelor or higher degree and those with college degree is higher than that of those with no more than a high school education respectively.It is not known whether a permanent staff has a worse self-development evaluation than a permanent staff.In terms of internal satisfaction,the external satisfaction of a person without a job is lower than that of an administrative manager.Compared with those with an annual income of 20,000 or less,the internal satisfaction of those with an annual income of 20,000-30,000 and 40,000 or more is higher.In terms of external satisfaction,the external satisfaction of those without management positions is lower than that of those with management positions.Compared with the annual incomeless than 20,000 yuan,the annual income of 20,000-30,000 yuan and 40,000 yuan or morehave higher external satisfaction.Work stress feelings,from the level of education,compared with the high school education,the work pressure of undergraduate and above is higher.Taking the internal medicine as the control group,the work pressure of the preventive health department 1s lower.Compared with the clinical group,the work pressure of townships and towns is Iower.In terms of turnover intention,compared with the annual income less than 20,000 yuan,the intention is lower than those with annual income of 40,000 yuan or more.Conclusions and SuggestionsThe nursing staff of medical institutions in Shandong Province are mainly young and middle-aged women.Their education degrees are mainly on three-year education,bachelor and above,most of their profeesional title ismedium-grade professional title.Most of them have registered nurse qualification certificates.The length of service is 10 years or more,and most nurses work 40 hours or less.The nursing staff in Shandong Province have different scores in different dimensions of work experience,and the influencing factors are different,but ingeneral,the nursing staff is satisfied with their work and the turnover intention is lower.Overall,the income level and the presence or absence of administrative duties are important factors influencing the work experience,indicating that economic factors and job nature are the main factors affecting the job satisfaction of nursing staff.Based on this,we proposedt he following countermeasures.On the macro aspect,policy makers in the health administrative department 14,should issue policies.First,increasing the level of nursing human resources allocation,the proportion of medical care,and promoting the rational allocation of medical resources.Second,providing more career development and promotion opportunities and increasing the number of nursing staff and the sense of gain in the promotion of professional titles.Third,establishing a good atmosphere of,respecting medical care”in the whole society and improve the satisfaction of nursing work.On the hospital aspect,first,hospital administrators should strengthen humane care,listen to and adopt rationalization suggestions for medical staff,strengthen communication with nursing staff,regularly understand the work of nursing staff,do psychological counseling for nursing staff,and timely solve their work pressures.Second,hospital administrators should rationally arrange nurses’ working schedule,implement flexible work system reforms,and increase their freedom of time.Third,institutional managers should further improve hospital management related policies.Hospitals should actively explore reforming the compensation system and improve performance incentives,improve the income level of nursing staff and increase the sense of acquisition.Finally,hospitals should pay attention to staff,s career development plans and provide more training opportunities to mobilize their enthusiasm and increase job satisfaction.On the Personal aspect,on the one hand,nursing staff should actively study postgraduate and academic exchange activities,and strive to improve their own business level and quality,on the other hand,they must learn to face a variety of complex doctor-patient problems with a positive attitude.
