Background and PurposeAlthough coronary artery disease has been provided effectively preventive treatment,it is still the leading cause of death in developed countries,and its incidence is also rising in developing countries.It is one of the important public health problems that impact human health and life.Coronary artery calcification complicates coronary artery disease and further increases mortality.So,how to prevent and early diagnose of coronary artery calcification is of great significance,and the purpose of this paper is to explore the correlation between serum homocysteine concentration and coronary artery calcification.Methods405 asymptomatic patients who were undergo coronary artery angiography reconstruction(CTA)in The third people's Hospital in Liaocheng were enrolled in the study from January 2017 to March 2019.The calcification score was calculated by the integral measurement software based on patients' coronary CTA scan results.Referring to the Agatston coronary calcification grading method,Such a method was semiautomated to calculate a weighted sum of the area of coronary calcification,wherein each calcified area was multiplied by a local density factor determined by the Hounsfield unit(HU)of the calcium(0:0-129 HU;1:130-199 HU;2:200-299 HU,3:300-399 HU;4:>400 HU),Then multiplied the calcification area(in mm2)and finally added the scores of each coronary artery in all sections of the CT to obtain a total calcification score.According to the integration results,four groups were divided:0,1-99,100-399,and>400,indicating no calcification group,mild calcification group,moderate calcification group,and severe calcification group,respectively.Results1.Compared with the calcification group,the levels of serum Hcy,hs-CRP and HbA1c were lower in the non-calcified group(p<0.0001),and the incidence of hypertension and stroke was also relatively lower.Secondly,the plasma homocysteine level in the non-calcified group(8.01 ±9.71 vs 9.98±6.39 vs 19.07±7.16 vs 25.61 ±9.01,P<0.005)was significantly lower than that in the mild calcification group,the moderate calcification group,and the severe calcification group.2.There was a significant positive correlation between CACS level and serum Hcy level(r=0.631,p<0.001).In addition,the level of CACS was also positively related to age(r=0.178,p<0.001),triglycerides(r=0.314,P<0.001),HbA1c(r=0.554,P<0.001),and hs-CRP(r=0.541).),P<0.001),etc.3.After single factor regression analysis,CACS levels were significantly associated with smoking(beta=-79.530,95%CI-111.031 to-47.670,p<0.001),hypertension(beta=-93.633,95%CI-122.011 to-65.255,p<0.001),TG(beta=55.369,95%CI 38.988 to 71.750,p<0.001),HbAlc(beta = 94.123,95%Cl 80.253 to 107.922,p<0.001),hs-CRP(beta=29.408,95%CI 24.932 to 33.884,p<0.001)and Hcy(beta=10.716,95%CI 9.425 to 12.007,p<0.001).Further more,age,gender,and variables with p<0.05 after single factor regression analysis were included in the multiple linear regression equation,and Hcy was still associated with CACS levels(beta=6.380,95%Cl 5.214 to 7.547,p<0.001).4.Further analysis of ROC curve showed that Hcy could effectively predict the occurrence of coronary artery calcification.The best predictive effect was obtained at Hcy of 1.60?mol/L.The sensitivity was 97.7%and the specificity was 45.5%.ConclusionsOur study showed that Hcy is significantly associated with coronary calcification detected by CTA,and elevated Hcy is a strong predictor of coronary calcification.In clinical work,we can detect the Hcy level and stratify the risk of calcified coronary calcification in related patients,so as to take targeted prevention and treatment measures.
