Objective:To compared the effects of two surgical methods,including transversely malleable vascular patch angioplasty and longitudinal malleable vascular patch angioplasty,on postoperative upper and lower limb blood pressure in patients with the coarctation of the aorta(CoA).Methods:Retrospective analysis of clinical data of 13 patients with the postductal coarctation of the aorta from December 2016 to December 2019,Vascular patch angioplasty with longitudinal ductility in 8 patients and listed as a control group;5 patients underwent vascular patch angioplasty with lateral ductility and were included in the experimental group.The changes of blood pressure difference between the upper and lower limbs of the two groups were observed after operation and during follow-up,and comparison between groups was performed.Results:There were no deaths in 13 patients.There was no significant difference in CPB time,aortic occlusion time,ventilator assisted breathing time and ICU time between the two groups(P>0.05).The blood pressure difference between the upper and lower extremities of the control group was 19.50 ± 0.93mmHg,which was significantly lower than that of the preoperative 60±13.63 mmHg(P<0.01);The difference of systolic pressure between the upper and lower extremities in the experimental group was 6.80±2.86 mmHg,which was significantly lower than that before surgery,59.4±18.05 mmHg(P<0.01).The decrease of pressure difference between the upper and lower limbs in the experimental group was higher than the control group,and the difference was statistically significant(P<0.05).12 patients were followed up for 12 months.The blood pressure of the upper and lower extremities change in the experimental group were more than the control group,and the difference was statistically significant(P<0.05).Conclusion:Both vascular patch angioplasty with lateral ductility and longitudinal ductility are effective methods for treating the CoA.The transversely malleable vascular patch angioplasty can more effectively reduce the blood pressure difference between the upper and lower limbs than the longitudinal malleable vascular patch angioplasty.
