Objective:The clinical and renal pathological features of 202 patients with lupus nephritis(LN)were retrospectively analyzed,and the correlation between LN pathology and clinical manifestations,disease activity score and laboratory indexes was discussed,which could be used to provide reference basis for evaluating the clinical condition of LN patients,renal pathological type inference,early treatment and prognosis of LN patients.Methods:From April 2007 to November 2018,202 hospitalized patients with SLE and LN confirmed by renal biopsy in the Second Affiliated Hospital of Kunming Medical University and complete data were included.The correlation between renal pathology and clinical manifestations,disease activity score and laboratory indexes of LN patients were retrospectively analyzed.Results:1.The distribution of sex and age:Among 202 patients,172 cases(85.15%)were females,with an average age of 31.36±11.79 years old.30 cases(14.85%)were males,with an average age of 34.40±14.998 years old.The ratio of male to female was 1:5.73.2.The proportion of renal pathological types:Type IV was the most common.followed by type ?.type ? was rare,and type ? was not found3.The clinical manifestations of extrarenal injury:109 cases(53.96%)of blood system injury were the most common,followed by 104 cases(51.49%)were rash,and 6 cases(2.97%)of central nervous system injury were rare.There was no significant difference in the clinical manifestations of extrarenal injury among different pathological types(P>0.05).4.Laboratory and clinical indexes:(1)The incidence of microscopic hematuria.the incidence of aseptic pyuria,the incidence of hypertension,the incidence of anemia,platelet values,total cholesterol values,low density lipoprotein cholesterol values,renal function,24-hour urine protein,plasma albumin values,positive rate of anti-ds-DNA antibody,complement C3 and C4 level were significant differences among different pathological types(P<0.05),while the incidence of cylindruia,le-ukocyte values,lymphocyte values,triglyceride values,high density lipoprotein cholesterol values and immunoglobulin IgG,IgA,IgM level were not significant differences among different pathological types(P>0.05).(2)The abnormal rate of renal function damage in LN patients diagnosed by Cys-C is significantly higher than that of Scr and BUN(P<0.001,P<0.001).(3)TG is an independent risk factor for 24hU-Pr.The higher the TG level,the higher the 24 hU-Pr(P=0.038).5.Pathological score and SLEDAI-2K score:There were significant differences in AI values,CI values and SLEDAI-2K score among different pathological types(p<0.001).6.Correlation analysis of SLEDAI-2K score,test indexes and pathological score:(1)Scr,BUN,UA,24hU-Pr and SLEDAI-2K were positively correlated with AI values(p<0.05);complement C3,C4,immunoglobulin IgA,IgM,ALB,HB and PLT were negatively correlated with AI values(p<0.05).(2)Scr,WBC,LYM,HB,complement C3 and C4 were positively correlated with Cl values(P<0.05);immunoglobulin IgG,IgA,IgM were negatively correlated with Cl values(P<0.05)Conclusions:1.Type ? was the most common pathological type of LN,followed by type ?.2.There was a certain correlation between the pathological type of LN and clinical indexes,which has guiding value for early diagnosis,pathological type,early treatment and judging prognosis of LN.3.The abnormal rate of renal function damage in LN patients diagnosed by Cys-C is significantly higher than that of Scr and BUN.4.TG is an independent risk factor for 24hU-Pr.The higher the TG level,the higher the 24 hU-Pr.5.The pathological score of LN has a certain correlation with SLEDAI-2K score and test indexes,and the pathological activity of LN can be evaluated by comprehensive analysis.
