Obejectives1.To develop the medical-nursing combined care service needs questionnaire suitable to elderly residents with non-communicable diseases(NCDS),in order to provide the tool to evaluate the medical-nursing combined care service needs of elderly residents with NCDS.2.To describe the current status of health literacy(HL)and medical-nursing combined care service needs among elderly residents with NCDS in Guiyang;3.To analyze the influential factors in background material that affect HL and medical-nursing combined care service needs among elderly residents with NCDS in Guiyang;4.To explore the correlation between HL and medical-nursing combined care needs among elderly residents with NCDS in Guiyang,to provide the theoretical basis for the construction of care content and mode of medical-nursing combined care for various elderly residents with NCDS.Method1.The items of the medical-nursing combined care needs questionnaire were prepared by interviewing and expert consultation based on the theory of hierarchy of needs.2 rounds of questionnaire and consulting were performed.Using correlation coefficient,exploratory factor analysis,reliability and validity analysis to test the medical-nursing combined care needs questionnaire.2.A non-experimental,cross-sectional study,480 elderly residents with NCDS,were recruited,multistage sampled from ten communities in different districts in Guiyang,and were surveyed ultilizing general social questionnaire,scales on HL for patients with NCDS and medical-nursing combined care service needs questionnaire.The statistics data were uploaded and analyzed via Epidate3.1 software and IBM SPSS23.0 software.The statistics methods used in this study included descriptive statistics,ANOVA test,independent sample T-test,multiple step linear regression and correlation analysis.The general social characteristics,current status of HL and medical-nursing combined care service needs among elderly residents with NCDS were described by frequency,percentage,mean,standard deviation and so on.Independent sample T-test and ANOVA test were used to analyze the differences in demographic characteristics of HL and medical-nursing combined care service needs among elderly residents with NCDS.Pearson correlation anylysis was used to explore the correlation between HL and medical-nursing combined care needs among elderly residents with NCDS.Results1.The Cronbach' s coefficient of the medical-nursing combined care needs questionnaire was 0.886,and test-retest reliability was 0.806;S-CVI=0.946,I-CVI of each divension were 0.955,0.940,and 0.935;After factor analyzing and screening,3 divensions and 20 items were included,and the cumulative contribution rate of the factors was 72.683%.The questionnaire includes three dimensions:Living Care Service,Medical-nursing Care Service,Cultural and Recreational Care Service.2.A total of 500 questionnaires were distributed of which 480 elderly residents were responded validly(response rate 96%).3.13.8%elderly residents suffer from only a kind of NCDS;52.9%elderly residents suffer from two kinds of NCDS;25.6%elderly residents suffer from three kinds of NCDS;5.0%elderly residents suffer from four kinds of NCDS;2.7%elderly residents suffer from greater or equal five kinds of NCDS.The most diseases they suffer from are coronary disease(65.0%),hypertension(5 5.8%),diabetes mellitus(51.9%),gouty arthritis(33.1%),hyperlipemia(30.2%),other NCDS(10.4%),cerebral apoplexy(8.3%)and tumor(3.1%).4.The minimum value of HL among elderly residents with NCDS in this study was 24 points,while the maximum was 120 points.The mean score was(85.06±25.98)points.The scores for four dimensions of HL,information acquisition ability,health improvement willingness,communicative intereaction ability,economic support willingness,were respectively(32.79± 12.41)points,(14.12±5.23)points,(28.13±9.24)points,(6.87±2.95)points.The proportion for four dimensions of HL,in order from high to low,were economic support willingness(52.29%),health improvement willingness(49.79%),communicative intereaction ability(43.54%)and information acquisition ability(33.96%).5.The minimum value of medical-nursing combined care service needs among elderly residents with NCDS in this study was 34 points,while the maximum was 94 points.The mean score was(68.28±12.05)points.The scores for three dimensions of medical-nursing combined care service needs,Living Care Service,Medical-nursing Care Service,Cultural and Recreational Care Service,were respectively(16.68±4.57)points,(32.85±7.52)points,(18.76±3.80)points.6.Multivariate linear regression analysis showed education,current.variety of drugs,healthcare provider payment?main caregiver and occupation were influential factors of HL scores.Religion belief was an influential factor of medical-nursing combined care service needs scores.7.Pearson correlation anylysis showed that the score of HL and medical-nursing combined care service needs score was significantly positively correlated,and the correlation coeffient was 0.173,P<0.05.8.Multivariate linear regression analysis showed health improvement willingness was an influential factors of medical-nursing combined care service needs scores.9.The levels of comprehension to the questionnaires among elderly residents with NCDS in this study were understand(39.8%)?mostly understand(53.1%)?partly misunderstand(5.8%)and hardly understand(1.3%).Conclusions1.The medical-nursing combined care service needs questionnaire with good reliability and validity could be used to evaluate medical-nursing combined care service needs.2.The level of HL among elderly residents with NCDS in this study was not high,which was only 70.88 valued via hundred mark system.The proportion of HL was just 36.67 percentage.Univariate analysis showed that elderly residents with NCDS of male,age of 60-69 years,college graduate or above,teachers,married status,monthly income more than 8000,remaining balance,spouse caregiver,basic medical insurance for urban employees,medical history less than three years,sufferring from one kind of NCD and no drug had higher HL.3.The score of medical-nursing combined care service needs among elderly residents with NCDS in this study was 56.9 valued via hundred mark system.Univariate analysis showed that elderly residents with NCDS of no religion and medical history between three and four years had high medical-nursing combined care service needs.4.The score of HL and medical-nursing combined care service needs score was significantly positively correlated.
