Objective:To investigate the short-term therapeutic effect,safety,and the feasibility for colorectal cancer by Natural Orifice Specimen Extraction(NOSE)with conventional laparoscopic colorectal resection(LCR),expound the clinical applicability of Natural Orifice Specimen Extraction(NOSE).Methods:According to the inclusion of standards and exclusion criteria,80 cases of colorectal cancer patients with surgical treatment in the Second Affiliated Hospital of Kunming Medical University included were divided into control group receiving conventional LCR treatment with 58 cases,and the other 22 cases were divided into study group receiving NOSES.Comparison of surgical data between the two groups of patients and indicators of preoperative and postoperative clinical tests,immunity and inflammatory factors.Results:There was no significant difference in the time of operation and the amount of bleeding between the two groups(P>0.05,both),but the length of hospital stays and postoperative exhaust time in the study group were significantly shorter than the control grou.simultaneously,the same as the number of cases of pulmonary/oral infection and the number of unplanned relieve pain cases more than 24 hours after operation(P<0.05).The clinical immunologic indexes IgA and IgM were lower than the preoperative ones at 3 days after operation in both groups,but the decrease was smaller in the study group than in the control group.CD3+,CD4+,and CD4+/CD8+were all higher than preoperative,but the degree of increase in the study group was significantly lower than in the control group(all P>0.05).The index of inflammatory factors in 3 days after operation of the two groups was higher than that of preoperative CRP,IL-6,and TNF-?,but the degree of increase in the study group was significantly lower than that in the control group.Conclusion:Based on the latest domestic and International Studies and the results of this study,it can be concluded that total laparoscopic surgery for colorectal cancer by natural cavity as a new type of minimally invasive surgery for colorectal cancer,under the premise of strict control of its indications,It has the advantages of having a small surgical burden,good postoperative recovery,and relatively few complications in the near future.At the same time,it is also in line with the beauty concept of the general public and reduces the risk of postoperative adverse psychological cues.It is worth further in-depth study and promotion in clinical practice.
