Objective:Through the analysis of peripheral blood of Regulatory T cells(Tregs)and its specific transcription factor fork/wing shape helix family transcription factor 3 mRNA(FoxP3 mRNA)expression and the levels of Transforming Growth Factor-?1(TGF-?1)and interleukin-10(IL-10)in patients with chronic hepatitis B(CHB),and to understand the changes in peripheral blood Treg cells,FoxP3 mRNA expression,and the levels of related cytokines TGF-?1 and IL-10.And their correlation between these indexes and HBV replication level and liver function indicators such as alanine aminotransferas(ALT),aspartate aminotransteras(AST),and total Bilirubin(TBIL)was discussed.Methods:In this study,126 HBeAg-positive chronic hepatitis B patients were selected from the outpatient department of infection of The First Affiliated Hospital Of Kunming Medical University from October 2016 to October 2018,including 78 males and 48 females.Twenty-two were normal physical examination people in our hospital as health control.Among them,14 patients were male,8 patients were female.By flow cytometry(FCM)to detect peripheral blood Treg cells accounted for the proportion of CD4+T cells;The expression level of Foxp3 mRNA in peripheral blood was determined by real-time fluorescence quantitative reverse transcription polymerase chain reaction(RT-qPCR).Finally,we can colonize Mars within Mars Mars group by enzyme linked immuno-immunosay(ELISA)assay to detect IL-10,TGF-?1 levels.HBV markers including HBsAg,HBsAb,HBeAg,HBeAb and HBcAb were determined by ELISA.Serum HBV-DNA load was determined by real-time PCR(RT-PCR)analyzer.Automatic biochemical analyzer was used to detect alanine aminotransferase(ALT),aspartate aminotransferase(AST)and total bilirubin(TBIL)and other liver function indexes.Spss17.0 statistical software package was used for statistical analysis.Results:The proportion of males in 126 HBeAg-positive CHB patients was 61.9%(78/126),the proportion of females was 38.1%(48/126),the average age was 34.43±8.15 years,and the average level of HBV-DNA was 7.66(±0.91)×107 IU/mL,the baseline level of HBV-DNA in all patients was greater than 1 X 105 IU/ml,and the baseline level of HBV-DNA was greater than 107 IU/ml accounted for 81.7%(103/126).There were significant differences in ALT,AST and TB between the normal control group and the chronic hepatitis B group(P<0.001).The expression levels of Treg cells and their specific transcription factors Foxp3 mRNA and the related cytokines TGF-?1 and IL-10 in patients with chronic hepatitis B were significantly higher than those in the normal control group(P<0.001).The levels of Treg cells and related transcription factors and cytokines in patients with chronic hepatitis B with different viral loads were significantly higher than those in the low-viral group(P<0.001).There were no significant differences in the levels of Treg cells and related transcription factors and cytokines in patients with chronic hepatitis B with different degrees of liver damage.There were no correlations between serum levels of Tregs,Foxp3,TGF-?1 and IL-10 in patients with chronic hepatitis B(ALT,AST and TBIL)(P>0.05).Treg cells were significantly positively correlated with Foxp3,TGF-?1,IL-10,and were statistically significant(P<0.001).The levels of Tregs,Foxp3,TGF-?1 and IL-10 were significantly positively correlated with HBV-DNA levels,and were statistically significant(P<0.01).Conclusion:The expression levels of Treg cells and their specific transcription factors Foxp3 mRNA and the related cytokines TGF-?1 and IL-10 in patients with chronic hepatitis B were significantly higher than those in the normal control group.There was a significant positive correlation between Treg cells and Foxp3,TGF-?1,IL-10,and the levels of Treg cells and their specific transcription factors Foxp3 and related cytokines TGF-?1,IL-10 and HBV-DNA load of HBV replication.There is a significant positive correlation.This indicates that Treg cells and their specific transcription factors and related cytokines are associated with HBV infection and are directly related to HBV replication levels.Increased Treg cells and their specific transcription factors and related cytokines may inhibit cellular immunity,attenuate the clearance of hepatitis B virus,and lead to chronic hepatitis B.Therefore,further study on the changes of peripheral blood Treg cells and their specific transcription factors and related cytokines in patients with chronic hepatitis B and their role in the development of chronic hepatitis B,can explain the pathogenesis of chronic hepatitis B and clinical treatment strategies provide a new theoretical basis.
