Objective To investigate the therapeutic effects of edaravone combined with salmeterol fluticasone in patients with COPD,including the intervention effects on oxidative stress,sputum IL-33,serum SP-D and Th17/Treg.Method From November 2018 ~ October 2019,98 patients with acute exacerbation of COPD in the Hospital were selected as the research objects,and they were divided into two groups by experimental method,49 cases each.There were 30 males and 19 females in the observation group,aged(66.52±6.31)years old,and 32 males and 17 females in the control group,aged(67.02±6.62)years old.Both groups were treated with conventional symptomatic treatment,such as antitussive,expectorant,antiasthmatic,oxygen inhalation,anti infection,etc;on this basis,the control group was treated with salmeterol fluticasone,and the observation group was treated with salmeterol fluticasone combined with edaravone,both groups were treated continuously for 2 weeks.Compared the total effective rate of treatment,the incidence of adverse reactions,the number of acute attacks,the rate of re-admission during treatment,and blood gas analysis indicators,oxidative stress indicators,,Induced sputum IL-33,serum SP-D levels,T lymphocyte subsets,and Th17/Treg before and after treatment,followed up for 3 months,compared the quality of life of the two groups.Result(1)The total effective rate was 93.88% in the observation group and higher than the control group’s 77.55% after treatment,which was statistically significant(P<0.05);(2)The difference in Pa O2 and Pa CO2 levels in the two groups before treatment with no significant(P>0.05);after 2 weeks of treatment,the levels of Pa O2 and Pa CO2 in the two groups were improved compared with those before,and the observation group was better than the control group with statistical significance(P<0.05);(3)The difference in SOD,MDA and NO levels was no significant in the two groups before treatment(P>0.05);after 2 weeks of treatment,the levels of serum SOD,NO,and MDA improved compared with those before in the both groups,and the observation group was better,with statistical significance(P<0.05);(4)The difference in Induced sputum IL-33 and serum SP-D levels between the two groups before treatment was no significant(P>0.05);after treatment for two weeks,the levels of Induced sputum IL-33 and serum SP-D were improved in the two compared with those before,and the observation group was lower than the control group,which was statistically significant(P<0.05);(5)The difference in CD3+,CD4+,CD8+,CD4+/CD8+ between the two groups before treatment was no significant(P>0.05);after 2 weeks of treatment in the two groups,CD3+,CD4+,CD4+/CD8+,and CD8+were improved compared with those before treatment,and the improvement in the observation group was better with statistical significance(P<0.05);(6)The difference in Th17 cells,Treg and Th17/Treg between the two groups was no significant(P>0.05);after two weeks of treatment,Th17 cells,Th17/Treg,and Treg IV were improved in the two groups compared with those before treatment,and the improvement in the observation group was better,and with statistical significance(P<0.05);(7)At the 2 weeks of treatment,the incidence of adverse reactions in the observation group was 12.24% compared with the control group’s 8.16%,with no significant(P>0.05);(8)Followed up for 3 months,counted the number of patients with acute exacerbation and the rate of readmission within 3 months after discharge;The number of acute exacerbations and readmission rates in the observation group were lower than the control group,and with statistical significance(P<0.05);(9)The difference in the quality of life scores before treatment was no significant between the two groups(P>0.05);after 3 months of treatment,the quality of life scores of the two groups were lower than those before treatment,and the observation group was lower,and with statistical significance(P<0.05).Conclusion Edaravone combined with Salmeterol and Kazon in the treatment of chronic obstructive pulmonary disease can effectively improve blood gas analysis,immune function,regulate IL-33,serum SP-D level and th17 treg through anti-inflammation and anti-oxidation,to reduce the number of cases of acute exacerbation and readmission rate,improve the quality of life of patients,the effect is more significant.
