Background:Autologous fat transplantation is a clinical technique in which adipose tissue is taken out by a certain liposuction method,purified in vitro and replanted in the same body to treat tissue defect depression and dysplasia.Autologous fat transplantation undoubtedly has a good prospect in plastic repair,especially in facial reconstruction,due to its advantages of no immune rejection,easy availability,repeatability and shaping.However,according to current reports at home and abroad,autologous fat transplantation is not absolutely safe.A series of intraoperative and postoperative complications occurred in autologous fat transplantation which due to the problems of fat purification and improper operation methods during the operation.Among them,fat embolism caused by blockage of blood vessels of important organs by free fat drops is especially serious,which may cause sudden blindness,shock,hemiplegia and even death.At present,there is no effective prevention and treatment method.For an orthopedic surgeon,how to improve the treatment method of autologous fat continuously,reducing the content of free fat drops in the transplanted fat,reducing or preventing the occurrence of fat embolism from the aspect of operation,and perfect the autologous fat transplantation technology is of great importance.Purpose:Through experiments,the effects of four different fat tissue treatment and purification methods including static precipitation,centrifugation,cotton pad adsorption and modified fat treatment method on the removal degree of free fat droplets are explored and compared,so as to provide a more convenient,safe and efficient method for the selection and treatment of fat in autologous fat transplantation.Method:Through two experimental methods of filter paper grid counting detection and centrifugation detection,the content of free fat drops in the purified fat graft treated by four different autologous fat tissue treatment,namely,static precipitation,centrifugation,cotton pad adsorption and modified fat treatment method,are compared to evaluate the removal degree of free fat drops by different fat treatment methods and evaluate the advantages and disadvantages of different purification treatment methods.Result:In the filter paper grid counting and detection,adipose tissue is treated and purified by four methods,namely,standing precipitation method,centrifugation method,cotton pad adsorption method and modified fat treatment method respectively.A quantitative fat mixture is dropped on the filter paper and the diffusion range of each group of grease is observed,and the number of squares occupied by grease under different fat treatment methods is counted by using a number of squares method.The results were as follows:static precipitation method 119.00±3.65,centrifugation method 83.75±4.65 cotton pad adsorption method 78.75±5.72,modified fat treatment method 74.50±5.80.The difference between the groups was statistically significant(P<0.05).In the centrifugal measurement and detection,the fat mixture purified by the four fat treatment methods discards the oil layer and the liquid layer,and then undergoes centrifugation,and the oil volume of each purification method after centrifugation is calculated.The results were 3.57±0.83 ml for static precipitation,1.28±0.32 ml for centrifugation,0.68±0.13 ml for cotton pad adsorption and 0.40±0.08 ml for modified fat treatment.The difference between the groups was statistically significant(P<0.05).Conclusion:To sum up,the two groups of experiments demonstrated the efficacy and superiority of the improved fat treatment method in removing free fat drops of autologous fat.The experimental results show that compared with static precipitation method,centrifugation method and cotton pad adsorption method,the modified fat treatment method has more advantages in removing free fat in autologous fat transplantation.
