Objective:In this experiment,the affected teeth with wedge-shaped defects were filled with resin by two-step self-etching bonding system.After applying the treatment agent,the tooth surface was brushed with the air pressure of 0.1MPa,0.3MPa and0.4MPa respectively,then the adhesive was coated,and the tooth surface was blown with the air pressure of 0.1MPa,and finally the resin was filled layer by layer.The clinical effects at the third,sixth and twelfth months after filling were observed and compared in order to find the best value of gas blowing pressure to provide basis for clinical operation.Methods:According to the order of outpatient visits,45 patients with wedge-shaped defects were randomly divided into three groups:conventional pressure group?0.1MPa group?,high pressure group?0.3MPa group?and highest pressure group?0.4MPa group?,with 45 teeth in each group.All the affected teeth in the three groups were treated with a two-step self-etching bonding system.First,a layer of mineralized dentin was removed from the defect surface with a slow mobile phone,and then a small bevel was prepared at the edge of the enamel cavity.then sterilized with alcohol cotton balls,moisturized,and then coated with a treatment agent for 20s.Then calibrate the barometer and blow the three groups of teeth with different air pressure:the conventional pressure group uses 0.1Mpa gas blowing pressure;the high pressure group uses 0.3Mpa gas blowing pressure;the highest pressure group uses 0.4MPa gas blowing pressure.Gas blowing method:blowing SE BOND treatment agent 5S,blowing distance is 1cm.After blowing,all the three groups were coated with a layer of SE BOND adhesive,and 0.1MPa pressure was used to blow the teeth again until the film was no longer flowing.Then light-cured 10s was applied to the three groups of teeth,then Filtek TMZ350XT composites was used to fill the cavity,Light curing for 20seconds.Finally,Modify the shape and polish the resin.Oral health education was given to the patients after filling,and the patients were revisited at the third,sixth and twelfth months after operation.the surface morphology,marginal closeness,marginal coloration,retention and secondary caries of the three groups were recorded at the third,sixth and twelfth months.Finally,the experimental results were statistically analyzed.Results:1.At the third month after filling,only one tooth in the 0.1MPa group was grade B,and all the other fillings were grade A.There was no significant difference in satisfaction rate among the three groups,and there was no significant difference in total satisfaction rate among the three groups.2.At the sixth month after filling,one of the affected teeth in the 0.1MPa group showed marginal staining and discoloration infiltrated into the dental pulp,one tooth was discontinuous and the dentin was exposed,and one tooth restoration fell off,which was classified as grade C.All the affected teeth in 0.3MPa group were A grade.In 0.4MPa group,one of the affected teeth was stained at the edge,and the discoloration was recorded as grade B,the surface shape of the filling was discontinuous,and the dentin exposure was recorded as grade C.There was no significant difference in satisfaction rate among the three groups,and there was no significant difference in total satisfaction rate among the three groups.3.At the twelfth month after filling,one of the affected teeth in the 0.1MPa group showed marginal staining,but no infiltration into the dental pulp was classified as grade B.The filling bodies of two affected teeth fell off;two of the affected teeth showed marginal staining and infiltrated into the dental pulp;the surface morphology of two affected teeth was discontinuous and the dentin was exposed;the gaps between the restoration and the teeth of three teeth were visible and the probe could be explored;they were recorded as grade C.In 0.3MPa group,one of the affected teeth showed marginal staining and was not infiltrated into the dental pulp as grade B,the surface shape of the affected tooth was discontinuous,the dentin exposure was marked as grade C,and the scores of other indexes were grade A.In 0.4MPa group,the filling of one tooth fell off,the edge of one tooth was stained and infiltrated into the dental pulp,and the surface morphology of two teeth was discontinuous and the dentin was exposed,which was classified as grade C.There was no significant difference in satisfaction rate among the three groups.Among the three groups,the total satisfaction rate was the highest in the 0.3MPa group,followed by the 0.4MPa group and the lowest in the0.1MPa group.There was a significant difference in the total satisfaction rate between the 0.3MPa group and the 0.1MPa group,but there was no significant difference among the other groups.Conclusion:1.The purpose of this study was to observe and compare the clinical effects of the three groups at the third,sixth and twelfth months after filling with air pressure blowing agent in 0.1MPa group,0.3MPa group and 0.4MPa group.It was found that there was no significant difference in marginal adhesion,marginal coloring,surface morphology,secondary caries and retention.3.At the third and sixth months,there was no significant difference in the total satisfaction rate among 0.1MPa group,0.3MPa group and 0.4MPa group.At the time of revisit in the twelfth month,There was a significant difference in the total satisfaction rate between 0.3MPa group and 0.1MPa group,0.3MPa group was better than 0.1MPa group,and 0.3MPa group was better than 0.4MPa group in terms of total satisfaction rate.Therefore,it is recommended to use 0.3MPa air pressure blowing agent to increase the satisfaction rate of clinical filling and improve the adhesive properties of the resin.
