Objective: this study was a meta-analysis of the clinical efficacy of posterior sacroiliac screw fixation and anterior sacroiliac joint plate fixation.By collecting domestic and foreign literature and extracting data,statistical methods were used to analyze whether there was a statistical difference in the clinical efficacy of the two surgical programs,so as to provide references for clinicians and patients from the perspective of individuals,hospitals and society,so as to select the best surgical program.Materials and methods: China National Knowledge Infrastructure(CNKI),Pubmed,Cochrane Library,viper database,wanfang database and other databases were used to retrieve literatures,and relevant articles on sacroiliac joint dislocation,unstable pelvic fractures,crescent pelvic fractures and sacroiliac screws were retrieved.English will retrieve Unstable pelvic fracture,Sacroiliac be dislocation,Sacroiliac screw,Sacroiliac be anterior plate,joint as keywords or keyword.Chinese retrieval of sacroiliac joint dislocation,unstable pelvic fractures,crescent pelvic fractures,sacroiliac screw and other joint as the key words or subject words.Data related to the included literature were collected and plotted into a table,and these data were analyzed by Review Manager 5.3.Results: 24 Chinese and English literatures were included in this meta-analysis,including 8 prospective studies and 16 retrospective studies.A total of 1471 fractures were treated with percutaneous sacroiliac screw fixation in 741 cases and 730 cases with anterior sacroiliac joint plate fixation.The included indicators were operative time,postoperative fever time,length of hospital stay,excellent and good rates of Matta and Majeed scores,incidence of postoperative complications,intraoperativeblood loss and incision length.The results showed that percutaneous internal fixation with sacroiliac screws had obvious advantages over anterior sacroiliac joint fixation in the treatment of sacroiliac joint dislocation,and the differences were statistically significant(P < 0.05).Conclusion: meta-analysis of the included literature shows that percutaneous sacroiliac screw internal fixation has obvious advantages in the treatment of sacroiliac joint dislocation compared with the anterior sacroiliac joint plate,and can reduce intraoperative blood loss,operative time,length of stay,incision length and postoperative complication rate.Increase the rate of Matta score and Majeed score.
