Characteristics And Direct Medical Costs Of Inpatients With Self-poisoning At Emergency Room Of A General Hospital

ObjectiveSuicide cause huge economic burden to individuals,families and society.In order to explore the characteristics of suicide attempters and their financial burden,to promote social concern about suicide and reduce its rate,this study investigated the self-poisoning patients found in the emergency department of a general hospital in Jingzhou City,Hubei Province.The research objectives are follows:1.To explore the characteristics of self poisoning patients found in emergency room of a general hospital,in Jingzhou City,Hubei Province(1)To explore the general characteristics of self and other types of poison.(3)To analysis the treatment and recovering conditions of self-poisoning patients.2.To assess the direct medical costs of self-poisoning patients and defind its main factors.MethodsCollected the basic information and direct medical costs of self-poisoning patients found in a general hospital in Jingzhou City from January 1,2015 to December 31,2019,The data were collected by the code of International Classification of Diseases-Tenth Revision(ICD-10)in Hospital Information System(HIS).Excel and SPSS23.0 software were used for statistical analysis,descriptive analysis,univariate analysis,correlation analysis and multiple linear regression analysis were used to find out the main factors that affected the direct medical costs.Results1.The characteristics of self poisoning patients found in emergency room of a general hospital,in Jingzhou City,Hubei Province(1)The general characteristics:The average age of 406 cases was(50.13±18.25)years old,and the male to female ratio was 1:1.48.65.27%belonged to rural household registration,1.97%had history of attempted suicide and 16.01%had a mental disorder,including 50.82%of depression and 31.5%of schizophrenia.(2)The characteristics of patients in different subgroups:?The average age of 212 patients suicide by pesticide poisoning was(53.45±16.20)years old,male to female ratio was 1:1.03,80.84%belonged to rural household registration;Organophosphorus pesticide was the main type of poison;6 of 16 patients had problem of depression,the same as schizophrenia.;?The average age of 150 patients suicide by intentional drug overdose was(43.74±18.82)years old,male to female ratio was 1:2.49,and 54.67%belonged to the urban household registration.In 47 patients with mental disorders,there were 24 patients had depression and 13 had schizophrenia.Benzodiazepine was the main type of drug;?The average age of 44 patients suicide by taking other poisons was(56.31±19.63)years old,the gender ratio(men:women)was 1:1.65,59.09%belonged to rural household registration,and 2 patients had depression.The most common poisons were rat poison products..(3)The patients' treatment and recovery conditions:84.91%patients suicide by self-poisoning got better,10.22%did not recover,and 4.87%died.The average age of the recovered patients was(48.96±17,87)years old,and the death was(62.90±19.77)years old,the difference between them was significant(P<0.01);38.92%patients could see the doctor within 2 hours,the proportion of patients be charged in through referral was 41.61%;the peak periods of pesticide poisoning were February and May,for intentional drug overdose were May and July,and for other poisons was August.2.The total direct medical costs of 406 self-poisoning patients were 4,535,500 RMB,per capita costs(M)were 5,068.50 yuan;Age,length of hospitalization and hemoperfusion were common factors that affect the direct medical costs.Factors that affect the costs of pesticide poisoning also included ventilation treatment and admission to the ICU,the factors that affect the costs of overdose also include dmission to the ICU.Conclusion1.The attempted suicide was May.(2)The characteristics of patients in different subgroups:In pesticide poisoning attempted group,the number of rural patients and elder was higher,organophosphorus was the most common type;In intentional drug overdose group,there were more urban patients,benzodiazepines(BZD)were the most common overdose drugs,the proportion of young people was the highest among all subgroup;In the other tyes of toxicants group,there were more rural patients,the most common toxicants were rat poison products.(3)The patients' treatment and recovery conditions:Most patients could get better after treatment,4.87%patients died;The patients who died were older;The proportion of patients be admitted through referral was high.2.The direct medical costs of 406 self-poisoning attempted patients were 4,535,500 RMB.The main factors that could affect it included the tye of poison,age,length of hospitalization and treatment method?The study found that the rural area had a higher incidence of attempted suicide by self-poisoning and the direct medical costs caused a heavy economic burden on families.Our society should pay more attention to suicide prevention.The
