Objective:To observe the effect of cloud drug "qilongtian" on lung function and airway remodeling related factors in patients with chronic obstructive pulmonary disease(COPD)in the stable period,and to provide a clinical trial basis for enriching the intervention means in COPD in the stable period.Methods:Using the method of randomized concurrent controlled clinical trial,it is planned to include 72 patients with standard COPD at stable period into the experimental group and the control group by stratified random method.The control group is given the basic treatment of western medicine:oxygen therapy and budesonide formoterol powder inhalation agent.The experimental group took "qilongtian" granule(1 time/bag,3 times a day,warm after meals)on the basis of western medicine,and the observation course was 3months.The changes of lung function(FVC,FEV1%,FEV1/FVC),TGF-?1 and MMP-9 in serum were compared before and after treatment.Meanwhile,the liver and kidney function,coagulation function,and adverse drug reactions were observed to investigate the effect of "qilongtian" on lung function and airway remodeling.Results:(1)The baseline analysis:into the group participants included 61 cases of patients,fall off and eliminate 9 cases,effective cases in 52 cases,26 patients of test group and control group in 26 cases,treatment of the former two groups of patients' age,gender,grade of GOLD distribution and curative effect index(FVC,FEV1,FEV1/FVC%,TGF-beta1,MMP-9),no significant difference(P>0.05),comparable.(2)FVC:compared within the group,FVC level in the experimental group after treatment was significantly different from that before treatment(P<0.05),FVC level in the control group before treatment was significantly different from that after treatment(P<0.05),and FVC level in the two groups after treatment was significantly different from that before treatment(P>0.05).(3)FEV1%:compared with the level of FEV1%before treatment,there was a significant difference in the experimental group after treatment(P<0.05),and no significant difference in the control group after treatment compared with the level of fev1%before treatment(P>0.05).there was no significant difference in fev1%level between groups(P>0.05).(4)FEV1/FVC:there was no significant difference in FEV1/FVC level in the experimental group after treatment compared with that before treatment(P>0.05),and no significant difference in FEV1/FVC level in the control group after treatment comparedwith that before treatment(P>0.05).there was no significant difference in FEV1/FVC level between groups(P>0.05).(5)Serum TGF-? 1:compared within the group,the level of TGF-? 1 in the experimental group after treatment was significantly different from that before treatment(P<0.05),and the level of TGF-? 1 in the control group after treatment was significantly different from that before treatment(P>0.05).(6)Serum mmp-9:compared with that before treatment,there was a significant difference in mmp-9 level in the experimental group after treatment(P<0.05),and no significant difference in mmp-9 level in the control group after treatment compared with that before treatment(P>0.05).(7)Safety indicators:there was no significant difference in liver and kidney function(AST,ALT,BUN,Scr),coagulation function(PT,APTT,TT,FIB)between the first two groups(P>0.05).There were no significant differences in AST,ALT and Scr levels in the experimental group after treatment compared with before treatment(P>0.05),significant differences in BUN levels in the experimental group after treatment compared with before treatment(P<0.05),and no significant differences in BUN levels in the control group after treatment compared with before treatment(P>0.05).After treatment,there was no significant difference in the APTT and TT levels in the experimental group compared with before treatment(P>0.05),the PT and FIB levels in the experimental group after treatment were significantly different from before treatment(P<0.05),and there was no significant difference in the PT and FIB levels in the control group compared with before treatment(P>0.05).Conclusion: "Qilongtian" can stabilize and improve lung function of COPD patients at stable period to a certain extent.Its mechanism may be related to reducing serum TGF-?1and MMP-9 levels,thus inhibiting or delaying the formation and development of airway remodeling in the course of COPD.It is of high safety and has the value of further research and promotion.
