Objective: By observing the imaging of white matter in people with sleep apnea hypopnea index less than 5,and the changes of their cognitive function,to investigate whether people with AHI<5 have white matter changes and cognitive function decline, and possible factors lead to the change. Methods: 101 patients were involved in this research.Collecting their general data, various routine biochemical indicators, polysomnography(PSG),head MRI,and other neuropsychological cognitive function assessment(MOCA)of 49 patients.Divided into the following 5 groups according to different AHI intervals: 0?AHI<1,1?AHI<2,2?AHI< 3,3?AHI<4,4?AHI<5,the white matter was scored by the modified Scheltens scale, and AHI was compared with white matter and cognition by single factor analysis of variance There is a significant difference in the relationship of P <0.05.Results:1.All AHI groups,in the comparison of gender, age,BMI,hypertension, diabetes, smoking, drinking, maximum apnea time, minimum blood oxygen saturation, oxygen desaturation index, average blood oxygen, platelets, fasting blood glucose, triglycerides, total cholesterol,high-density lipoprotein,low-density lipoprotein, and homocysteine, the oxygen desaturation index in five group is higher than the first four groups, and the oxygen desaturation index is significantly different between groups(P<0.05),when AHI is between 4-5,there is a significant difference in the oxygen depletion index of the population(P<0.05).2.There was no significant difference in the deep white matter score, periventricular white matter score, and total white matter score between different AHI groups(P>0.05);3.Compare the cognitive function between different AHI groups, delayed recall scoring in five group is lower than the first four groups, delayed recall has significant differences between groups(P<0.05),and the other indicators have no significant differences between groups;4.Compare the delayed recall values of the AHI 0-4 group and the 4-5 group, the difference was statistical significant(P<0.05).Conclusion:1.The white matter changes in patients with AHI < 5 were not sensitive in conventional magnetic resonance examination.2.The oxygen desaturation index in AHI between 4-5 was significantly higher and delayed recall was impaired, which may require early intervention and treatment.
