Objective:Orodeka Plex-v nickel-titanium file is a nickel-titanium device manufactured by a Sino-Italian joint venture company and newly put on the market in recent years,and at present,there are few clinical reports on the Orodeka Plex-v nickel-titanium file.In this study,clinical experiments were carried out for comparing the novel Orodeka Plex-v nickel-titanium file and the home-manufactured M3-PRO nickel-titanium file by the following means of recording the severity of pain and the incidence of pain after the clinical preparation of root canals,performing the experiments in vitro through simulating the root canals by resin,recording the preparation time took for single resin simulation,and comparing the forming capability of the Orodeka Plex-v nickel-titanium file and the M3-PRO nickel-titanium file,so as to provide clinical reference for the promotion and application of the novel Orodeka Plex-v nickel-titanium file.Methods:Clinical experiments:40 molars(19 maxillary teeth and 21 mandible teeth)underwent root canal treatment in this hospital from August 2019 to November 2019were selected,including 138 root canals,17 males,23 females,aged 16-76 years.All the cases were divided into an O group and a M group randomly according to the registration order.X-ray films were taken to evaluate the degrees of tooth damage for the patients of the two groups before the treatment,and root canals were prepared by respectively using Orodeka Plex-v and M3-PRO,and the canals were sealed by calcium hydroxide paste.During the subsequent visit after one week of the operation,the number of the patients with pain symptoms was recorded,the postoperative pain grades were evaluated,and root canal filling was carried out by using hot teething gel for the patients meeting the filling indications.Experiments in vitro:20 single curved resin simulated root canals of the same specification were selected,randomly numbered and divided into an OE group(number1-10)and a ME group(number 11-20).The root canals of the two groups were negotiated by using a No.15 K file,red ink was injected into the root canals,and the ink injected root canals were placed on the same fixed position,photographed with a camera to record the root canal images before the preparation,and washed to remove the ink by using normal saline.The root canals were prepared by respectively using Orodeka Plex-v and M3-PRO,during which EDTA gel was used for strengthening the lubrication.The resin root canals were washed by using normal saline at each moment of replacing instruments,and prepared to V2+for the OE group and to M2 for the ME group.The root canals were injected with blue ink after the preparation,placed on the same position,and vertically photographed by the same method to record the root canal images after the preparation.All the above operations were completed by one doctor,and the preparation time(excluding the time took for replacing instruments and flushing)of single root canal was recorded by another doctor.The root canal images before and after the preparation were overlapped by photoshop CS6,and the resin removal amount of 20observation points in the distance of 1-10 mm from the apical hole was calculated through Image Pro Plus6.0.The root canal offset and the center positioning capability were calculated,and the data was statistically analyzed by using SPSS21.0.Results:1.In the clinical experiments,no significant differences in number of the patients with pain symptoms and the incidence of pain were showed between the two groups after the preparation(P>/=0.05). 2.In the experiments in vitro:(1)the preparation time of root canals was not significantly different between the two groups(P>/=0.05);(2)the resin removal amounts on the internal wall of ME group in the distances of 4mm and 5mm from the apical hole were more than those in OE group,the significant differences were showed(P</=0.05);(3)the resin removal amount on the lateral wall of OE group in the distance of 4 mm from the apical hole was more than that in the ME group,and the resin removal amounts on the lateral wall of ME group in the distances of 8mm,9mm and 10mm from the apical hole were more than those in the OE group;the significant differences were showed(P</=0.05);(4)the root canal offset in the distance of 4mm from the apical hole in ME group is less than that in the OE group,the significant differences were showed(P</=0.05);(5)the difference in the center positioning capability was not statistically significant for the two groups(P>/=0.05);and the values on each observation point for ME group were more closer to 1 than those of the OE group.Conclusion:1.By contrast,both Orodeka Plex-v nickel-titanium file and M3-PRO nickel-titanium file good in flexibility and forming capability can effectively maintain the original shape of the root canals,and are good in root canal preparation effect,low in cost,difficulty to break,strong in elasticity and high in cost performance; 2.Orodeka Plex-v is relatively short in clinical preparation time,high in cutting efficiency,good in preparation effect for straight root canals and slightly curved root canals,and low in incidence of postoperative pain;3.home-manufactured M3-PRO is better in center positioning capability,lower in root canal offset after the preparation and good in preparation effect for moderate and severe curved root canals,and can effectively prevent the root canals from laterally perforating on the basis of a good root canal shape.Both the nickel-titanium files have certain clinical use values.The novel Orodeka Plex-v nickel-titanium file is worthy of clinical proportion and application.Clinicians should select the appropriate file according to the different situations of patients.
