Objective: To retrospectively analyze the expression difference of TLS in PTC and HT combined with PTC,and the relationship between TLS and the clinicopathological features of both, and to explore the possible role of TLS in the pathogenesis of PTC and HT-PTC.Method: 1?From March 2013 to August 2019,Selecting 40 cases of thyroid papillary carcinoma with Hashimoto's thyroiditis specimens and thyroid papillary carcinoma underwent thyroidectomy under in breast and thyroid surgery in the Second Affiliated Hospital of Nanhua University.2?Collect the clinical data of the patients, including their gender?age?lymph node metastasis?pathological classification?pTNM stage?TgAb and TPOAb levels.3?The pathological specimens of HT-PTC and PTC groups were tested for the expression of tertiary lymphatic structure in the two groups by immunohistochemistry(IHC)method. The components of TLS mainly include Tcells?B cells?DC cells?GCB cells?and HEV.Therefore,by marking CD3?CD20?CD21?Bcl-6?PNAD in tumor tissues to identifycorresponding T cells ? B cells ? DC cells ? GCB cells ? and HEV componenets, and were determind according to the IHC criteria.4?Acorrding to the expression of TLS in tumor,Setting the TLS positive as group A,and TLS negative set as group B.Then, analyze the exprssion difference of TLS in two groups of pathology, and explore the relationship between the expression of TLS and the clincopathological characteristics of patients.4?Acorrding to the expression of TLS in tumor,Setting the TLS positive as group A,and TLS negative set as group B.Then, analyze the exprssion difference of TLS in two groups of pathology, and explore the relationship between the expression of TLS and the clincopathological characteristics of patients.5?All statistical analyses were performed with the SPSS software version 26.0.The test level by alpha is equal 0.05,P-value less than 0.05 was statistically significant. Results: 1?T cells?B cells?GCB cells?DC cells and HEV expression rate in PTC tissues were 57.5%(23/40)?50.0%(20/40)?47.5%(19/40)?42.5%(17/40)? 45.0%(18/40),and in the HT-PTC tissues were 72.5%(29/40)?70.0%(28/40)?60.0%(24/40)?57.5%(23/40)?52.50%(21/40);2?The positive of TLS in HT-PTC and PTC tissues were 45.0%(18/40)?22.5%(9/40),and has significant difference(P<0.05);3?In the HT-PTC group, the lymph node metastasis rate of group A was lower than that of group B and difference was significant(P<0.05),the positive rates of serum TgAb and TPOAb concentrations were lower than that in group B,with significant difference(P < 0.05),group A compared with B patient gender?age?and TNM stage,no significant difference;4?In the PTC group, the positive rates of serum TgAb and TPOAb concentrations were higher than that in group B,with significant difference(P < 0.05),while there were no significant differences in gender?age?TNM stage and lymph node metastasis. Conclusions: 1?The expression of TLS may be negatively correlated with lymph node metastasis in patients with HT-PTC,and may inhibit tumor metastasis;2?The expression of TLS may decrease the level of TgAb and TPOAb in patients with HT-PTC,may inhibiting the further development of HT to PTC,and has no correlation with the age? gender and pTNM stage of the patients;3?The expression of TLS may increase the level of TgAb and TPOAb in PTC patients, and has no correlation with the age, gender, and pTNM stage of the patients.
